Plans for the Fort St. Vrain Nuclear Generating Station Attached is a copy of an Exercise Report Update on the August 5,1987, joint exercise of the offsite REP plans for the Fort St. Vrain Nuclear Generating Station. This Exercise Report Update has been prepared to include the results of the remedial drill conducted August 30, 1988, and the status of correction of the Areas Requiring Corrective Action. This Exercise Report Update and the exercise report dated February 3, 1988, which was transmitted to you on June 14, 1988, constitute the final report on the August 5,1987 exercise.
The Exercise Report Update was prepared by the Region VIII office staff of the Federal Emergency Management Agency (FEMA). Specifically, this report includes the results of the third remedial drill conducted to correct the Deficiency involving plume exposure emergency planning zone (EPZ) monitoring procedures and contains an update on the other areas that required corrective action by the State. The major delay in conducting the August 30, 1988 remedial drill was caused by the time needed to obtain Federal Communications Commission authorization for the State field monitoring teams to use a different radio frequency and to ensure that associated communications equipment was adequate.
Based upon the findings in the attached report, and in the report transmitted to you on June 14, 1988, the Colorado State and local offsite radiological emergency plans and preparedness remain adequate to provide reasonable assurance that appropriate measures can be taken offsite to protect the health and safety of the public in the event of an accident at the Fort St. Vrain Nuclear Generating Station and the 44 CFR 350 approval granted on January 29, 1982, remains in effect.

FOREWORD This report when combined with the initial biennial report from the August 5, 1987 exercise will constitute a final report.
The report is an evaluation of a remedial drill conducted August 30,1988. The scenario was designed to evaluate FEMA Objectives related to deficiency (87-D2) and to determine the status of ARCA(s)(87-A9,87-A10,87-A1.).
It represents a deviation from past reports in that it is cbjective based. Exercise results are associated with a given FEMA objective. Jurisdictions are discussed and evaluated within the context of an Objective. Exercise results are viewed with respect to such things as performance, decision mak!ng, demonstrating ability, operations, equipment, communications, etc. A clear, concise statement that the objective is " Met" or "Not Met" concludes the evaluation of each objective. Deficiencies and ARCA(s) are related to Objectives and identified or corrected as an essential part of the evaluation of each Objective.
There is a certain amount of carry over from the previous report which was written under the old criteria. Tables 1 and 2 in this report are updates of the original tables from the initial report.
In most other respects this report follows previously established conventions.

INTRODUCTION AND AUTHORITY On December 7,
1979, the President directed the Federal Emergency Management Agency (FEMA) to assume lead-role responsibility for all off-site nuclear power facility planning and response.
FEMA's immediate basic responsibilities in Fixed Nuclear Facility Radiological Emergency Response Planning include:
Taking the lead in off-site emergency response planning and in the review and evaluation of state and local government emergency plans, ensuring that the plans meet the Federal criteria set forth in NUREG-0654, FEMA REP-1, Rev.1 (November 1980).
Determining whether the state and local emergency response plans can be implemented on the basis of observation and evaluation of an exercise conducted by the appropriate emergency response jurisdictions.
Coordinating the activities of volunteer organizations and other involved Federal agencies.

3 1 EXERCISE OVERVIEW 1.1 BA OEGROUND On August 5,1987 a biennial off-site exercise was conducted by the State of Colorado, for the Fort Saint Vrain Nuclear Generating Station.
The exercise was evaluated by FEMA Region VIII. This biennial was to have completed a six-year cycle to evaluate off-site planning, preparedness and emergency response.
As a result of the August 5 exercise and subsequent to it, one medical and three remedial drills were conducted.
The drills were held as follows:
Medical Drill-September 30, 1987; Ingestion Pathway and Laboratory Remedial Drill-November 4, 1987; Public Alerting and Notification Remedial Drill-December 11, 1987; Plume Field Monitoring Teams and Communications Remedial Drill-30 August 1988.
Normally, remedial drills that result from full scale exercises are conducted within 120 days. The State of Colorado petitioned FEMA Region VIII for and was granted exercise extensions sufficient to obtain funding, procure and field test communications equipment. This equipment was required to correct communications problems associated with deficiency (87-D2).
Results of the August 30,1988 remedial exercise are contained wlRhin. This report when added to the Initial Biennial Report for the August 5,1987 exercise constitutes a Final Report.
1.2 EVALUATION CRITERIA 1
The August 30, 1988 remedial was conducted I.A.W. NUREG-0654/ FEMA-REP-1, Rev.1, Supp.1, and FEMA Guidance Memorandum EX-1.
Specific objectives were evaluated using FEMA's REP Exercise Manual and " Objective Based" Exercise Evaluation Methodology (EEM), dated 25 May 1988.
1.3 EXTENT OF PARTICIPATION The extent of participation was limited to those agencies of Colorado State I
government necessary to demonstrate the State's ability to correct Deficiency (87-D2).
l Areas Recommended For Corrective Action (ARCA)(S)(87-A9), (87-A10), (87-A11) were also reviewed to determine corrective action status.
1.4 FEMA OBJECTIVES TO BE DEMONSTRATED

The initial report (page 87) lists a summary of FEMA Objectives remaining to be i
met by jurisdiction. As of the publication of the initial report, FEMA Objectives (5, 8, l
- 20) were not met and an overall deficiency (87-D2) was identified.

Since the August 5,1987 biennial, FEMA has renumbered and/or rewritten their exercise objectives (Ref. FEMA EEM May 25,1988). See table (4.3) for a comparison of new vs. old objectives.

The August 30,1988 remedial drill was designed to include sufficient opportunity to demonstrate profielency in the following old FEMA Objective (s); (5) Demonstrate ability to communicate with all appropriate locations, organizations and field personnel, (8) Demonstrate appropriate equipment and procedures for measurement of airborne radiolodine concentrations as low as 10-7 uCl/cc in the presence of noble gases, (20) demonstrate ability to continuously monitor and control emergency worker exposure. These objectives were identified in the initial exercise report as " remaining to be met"(ref. p. 87, para 4.1).
FEMA Objective 5 in the initial report has become Objective 4 in the new EEM
" Objective-based" criteria. Only the number (i.e. 5 vs 4) of the objective changed; the language, the meaning and the substance of the objective-remained the same. New Objective 4 requires demonstration of the ability to communicate with all appropriate locations, organizations and field personnel.
FEMA Objective 8 was not changed. In the new EEM, Objective 8 is a verbatim copy of the old Objective 8. New Objective 8 requires demonstration of the appropriate equipment and procedures for the measurement of airborne radiolodine concentrations as low as 10-7 microcuries per ec in the presence of noble gases.
FEMA Objective 20 in the initial report has become FEMA Objective 6 in the new EEM " objective-based" criteria. Only the number of the objective changed; the language, the meaning and the substance remained the same. New Objective 6 requires demonstration of the ability to continuously monitor and control aciergency worker exposure.
NOTE: New FEMA Objective 7 was applied to the scenario by federal evaluators in an attempt to determine the current status of ARCA (87-A10).

2.5 DEFICIENCIES /ARCA'S REQUIRING CORRECTION l
' An overall defielency was identified based upon the number and importance of areas requiring corrective action identified with the plume field monitoring teams, the ingestion field monitoring teams and the ingestion sample lab. The following descriptions and recommendations support the deficiency as it relates to the ingestion field monitoring team demonstrations. Three ARCAs were reviewed to determine current status.

Description:==
The primary communication system between the field monitoring teams and the FCP did not function. This resulted in excessive delays in repositioning plume field teams to new

monitoring and sampling points and resulted in ineffective use of
]
the plume teams once they entered the field (NUREG-0654, II, F.1.d.).
Recommendation:
Supply. operable and effective equipment to communicate between the FCP and field monitoring teams. This will ensure proper direction and control of field monitoring teams to minimize radiation exposure while conducting field monitoring and sampling activities. Field team communicators need to be trained on proper use of the communications equipment and proper radio protocol.

Description:==
Both plume teams failed to demonstrate acceptable emergency worker exposure control. The single.1500 mR direct reading dosimeter was not appropriate for the 3 rem administrative exposure limit. A member from field team 2 was not wearing a permanent record dosimeter, and both members of field team 2 were deployed into the plume with possibly malfunctioning direct-reading dosimeters. All field team members were deployed without KI in their possession (NUREG-0654, II, J.10.e, K.3.a).
Recommendation: A review of emergency worker exposure control is needed, especially with regard to availability of permanent record t
dosimeters, chargers, and effective use and availability of suitable direct-read dosimeters for the 3 R administrative exposure limit. A review of the procedure for the issuance of KI to emergency workers is also needed.

Description:==
Significant errors were made by team 2 in the analysis of air sample results. The silver zeolite cartridge was loaded upside down, and calculations were not in accordance with the procedures (NUREG-0654, II, I.8, I.9).
Recommendation:
Specific training for the analysis of air particulate and radiolodine samples should be administered to potential field team members.

Recommendations: Pre-pack field team equipment into kits and consolidate briefings to hasten deployment of the plume monitoring teams.

Description:==
Direct radiation readings taken by team 2 did not incorporate ground level open and closed window readings to evaluate ground deposition (NUREG-0654, II, I.8).
Recommendations:
Procedures should include direct radiation readings to assure that ground level measurements are taken to allow evaluation of grand deposition.

Description Plume team I was not sure if the conversion factor provided on the air sample calculation sheets was correct for the instruments that were being used. Initially, they used a conversion factor which had 'e different value in the exponent. This could lead to orders of niagnitude differences in the calculated results (NUREG-0654, II, I.8, I.9).
Recommendation: Provide more training to.the field teams with regard to the proper use of the conversion factor in the training and l
perhaps include the derivation as a footnote on the calculation i
sheets.

8 3 EXERCISE EVALUATION The exercise evaluation included herein is organized by objective rather than by activity and/or location. The results of the remedial drill thus is provided by objective.
3.1 NEW OBJECTIVE FOUR (OLD OBJECTIVE 5)
" Demonstrate the ability to communleate with all appropriate locations, organizations and field personnel". Required to correct deficiency (87-D2).
In the initial exercis, 5 August 1987 the State of Colorado experienced difficulty e
in communleating between the FCP and dispatched Field Monitoring Teams.
The problem was identified has primarily hardware related and required a repositioned repeater to assist line of site communications equipment in difficult terrain.
As a result of the August 5,1987 biennial and subsequent deficiency (87-D2) the facility owner / operator Public Service Company Of Colorado financed the State of Colorado's purchase of a Meade Repeater to solve the field communications problem.
3.1.1 Equipment (Commun!eations)
The State of Colorado demonstrated during the August 30, 1988 remedial their ability to communicate between the FCP and operational field monitoring teams. All equipment was operational including the Meade Repeater, two-hand held radios (one assigned to each monitoring teatn), several backup hand-held radios and a base station located at the Forward Command Post. A trailer mounted Motorola Mt 500 backup system was available but it use was not required or demonstrated.
3.1.2 Communications Net Reference Colorado Radiological Emergency Response Plan (RERP) for Fort Saint Vrain, Section 5.0 Telecommunications, dated 7/88, rev. A, page 3 of 4. The State of Colorado Local Government Radio Network was used in support of the August 30,1988 remedial. The network is a j
lo-band VHF radio system consisting of several dozen base stations

(including one located at the FCP) and numerous mobiles. The net is I
used by the Colorado State Health Department (CDH), and the Colorado Division of Disaster Emergency Services (DODES) among i
others.
The State of Colorado local government radio network adequately supported the FCP dispatch field operations.

1 3.1.3 Field Monitoring Teams J

equipment and the communleations not was satisfactory. All required field transmission were made and received. However, on one occasion field team personnel attempted use of radios under and/or adjacent to power lines. Attention to the task at hand and not lack of procedures q
was the main contributing factor. NOTE: Continued, periodic hands on j
J training 'and/or use of field communleations equipment by CDH personnel is essential to maintain current levels of profielency.
.3.1.$ Forward Command Post Use of radios and knowledge of communications prociedures at the FCP was satisfactory. Suffielent operational radios were available to direct and control dispatched field monitoring teams. Radiation exposure of field - monitoring teams. was minimized during simulated field monitoring and sampling activities due to the FCP/CHD's use of radio communications.
3.1.5 Status of Defielency Deficiency (87-D2) as described in the initial exercise report (ref p. 41) is corrected. New FEMA Objective 4 (old 5) was met.
3.2 NEW OBJECTIVE SIX (OLD OBJECTIVE 20)
" Demonstrate the ability to continuously monitor and control emergency worker exposure". Demonstration of objective 6 was required to correct deficiency (87-D2) and used to determine current status of ARCA (87-A9).
3.2.1 Fleid Monitoring Teams Personnel on both field monitoring teams were knowledgeable and profielent in the wear and use of self reading and TLD type dosimeters.
3.2.2 Equipment (Dosimeters)
Personnel on both field monitoring teams reported for duty with TLD dosimeters. All team members were issued 3 self reading dosimeter (DCDV-138's 0-200MR, DOS-865's 0-1500MR, and CDV-742's 0-200R).
All dosimeters were zeroed and readings recorded on team checklists and the Emergency Worker Radiation Exposure Record, ER:Exp-1.

10 3.2.3 Operational Procedures (Dosimetry)
At the direction of the CDH/ Team Chief field team personnel read, recorded and reported individual member dosimeter reading approximately every 15 minutes. Exposure limits were established by the CDH/ Team Chief. 200 mrem readings required reassessment of the individual's need to remain in the radiation field. Individual exposure
!!mit was briefed at I rem (Whole Body).
FCP/CHD personnel monitored whole body exposure by periodically asking teams to check direct reading dosimeters. All readings were reported and recorded on Er:Exp-1 (Exposure Control Records). Potential personnel exposure was held as low as reasonably possible by limiting the time each team / team member spent in the plume. At the conclusion of field monitoring activities team members read their dosimeters upon plume exit and the
" Indicated Dose" recorded on the Emergency Worker Radiation

Exposure Record.
i 3.2.4 Operational Procedures (KI Issue) l The scenario did not require issue of KI to emergency workers. - The State of Colorado does stock and maintain a supply of Kl. However, the Department of Health's Emergency Response Operating Procedures do not require its issue.
3.2.5 Status of Deficiency Defielency (87-D2) is corrected as a result of this remedial, Emergency worker exposur control has been adequately addressed by the Colorado Department of Health. Comprehensive written operational procedures are in place. Field monitoring teams demonstrated profielency in the application of written procedures and the use of self reading and TLD type dosimeters.
3.2.6 Status of ARCA ARCA (87-A9) is corrected as a result of thisiemedial.
3.3 NEW OBJECTIVE SEVEN (OLD OBJECTIVE 7)
" Demonstration the appropriate equipment and procedures for determining field radiation measurements". Demonstration of objective 7 was required to determine the current status of ARCA (87-A10).
11 3.3.1 Field Monitoring Teams Plume field monitoring teams were dispatched to locate the projected plume and track its movement. While enroute each team monitored instrument meter scales to identify plume boundaries. Readings while inside the vehicle were taken with the detector held at a level above the lap. Upon initial detection of the plume each team. stopped their vehicle and performed exposure rate measurements with Eberline RO-5 A's.
At all locations monitored (outside the vehicle), readings were taken twice, approximately 3 feet and 3 inches from the ground.
Disposition surveys, were accomplished.during each exposure rate measurement. Readings were taken with the instrument " window open" (beta gamma), and the. instrument " window closed" (gamma). Beta calculations were performed as an integral part of the exposure rate measurements. Significant beta measurements would indicate direct contact with the plume.
All exposure rate measurements were recorded by team members and reported to the FCP.
3.3.3 Equipment (Radiac)
Field teams were lasued Eberline RO-5A's (0-200 mR/hr) for plume locating and tracking. Battery and source checks were performed by each team prior to field dispatch. All instruments worked satisfactory under field conditions. NOTE: Survey instruments can be protected from contamination by direct contact if they are placed in plastic bags j
and sealed prior to field dispatch.

12 3.4 NEW OBJECTIVE EIGHT (OLD OBJECTIVE 8)
" Demonstrate the appropriate equipment and procedures for the measurement of airborne radiolodine concentrations as low as 10-7 (.0000001) microcurie per cc in the presence of noble gases.
Demonstration of objective 8 was required to correct Deficiency (87-D2) and used to determine the current status of ARCA (87-A11).
3.4.1 Field Monitoring Teams Both field monitoring teams were issued appropriate equipment and supplies for airborne lodine sampling and sample measurement. All equipment was operational checked at the FCP prior to field dispatch.
Field monitoring team personnel were knowledgeable and profielent in their measurement of airborne radiolodine concentrations.
3.4.3 Equipmer.t (Air Sampling / lodine)
In addition to a portable generator, CDH/Fleid Monitoring Teams were
. supplied with RADeCO H809V-1 high volume air samplers, sample head (filter holders), Eberline E-520 portable survey meters, Hp-190 probes, silver zeollte cartridges. Other equipment and supplies required to support the collection, identification, evaluation and quantifying of airborne radioactive contaminants were issued (Reference Colorado,'
Department of Health's Emergency Response Operating Procedures Guide). All equipment was operational under field conditions, supplies I
were issued to meet scenario requirements and reserves seemed l
adequate.
3.4.3 Operational Procedures Specific, written operational procedures were available and used by both teams. Collection procedures were followed as outlined in CDH's Emergency Response Operating Procedures Guide. Both particulate and charcoal filters were placed in the filter holder correctly. A 15 cubic foot sample was taken, placed in the sample holder assembly and counted. Samples were properly handled, prepared, bagged, labeled and results reported to the FCP/CDH. Field airborne iodine sampling, sample measurements and subsequent calculations were performed correctly by both teams.
NOTE: Careful attention was given by each Team, Team Leader and
- t. e CDH/ Team Chief to limiting team member exposure. Air sampling pumps were operationally preassembled outside of the plume by both teams.
The initial exercise report contained a recommendation to

13-the sample. This procedure was fully implemented and satisfactorily demonstrated during this remedialin response to a previously identified ARFI.
3.4.4 Status of Deficiency (87-D2)
No errors were made by eith'er monitoring team during their analysis of air sample results.
Calculations were made in accordance with guidance from both the Colorado Department of Health, and the Facility operator.
Written guides /worksheets were consistent with (NUREG-0654, II, I.8, I.0). The silver zeolite cartridges were loaded correctly and in accordance with written instructions contained in the Colorado Department of Health's, Emergency Response Operating Procedures. Deficiency (87-D2) is corrected.
.S 3.4.5 Status of ARCA (87-A11)
Operational guidance and air sample calculation worksheets were available for Field Monitor Team use. Team #1 did find a worksheet containing an error. However, the error was noted the worksheet was determined to have been rescinded. A current and corrected worksheet was used and the resulting calculations were correct. The problem was briefed to the CDH/ Team Chief and a search' for other rescinded materials was made. The Team Chief deemed that no further action '
was required. ARCA (87-A11)is corrected.

VRAIN EXERCISE AND REMEDIAL DRILLS Table 1 provides a consolidated current summary of all deficiencies, and Areas Requiring Corrective Action (ARCA) associated with the August 5,1987 biennial and subsequent remedial drills. As of the publication of this report all deficiencies and some ARCA's resulting from the August 5,1988 biennial exercise have been corrected.
Remaining / uncorrected ARCA's in Table 1 are Identified by an asterisk. Uncorrected j
ARCA's are subjects for inclusion in the next biennial exercise.
NOTE: Correction of ARCAs 87-A1, 87-A5, 87-A6, and 87-A15 was based on written updates / changes and were not field verified.
FEMA may request field verification at next appropriate biennial exercise.

40 4.2 FEMA OBJECTIVES TRACKING CHART (UPDATE)
Table 2 represents the current status of the 6-year exercise cycle' ending December 31, 1987. Status has been updated as a result of the August 30,1988 Field Plume Tee.m Monitoring Remedial. Table 2 provides a comprehensive tracking system of FEMA Objectives, NUREG-0654 Reference Elements, Exercise Objectives, Jurisdictional Responsibilities, Exercise Dates, Deficiencies, Areas Requiring Corrective Action (ARCA) and the Date Specific Objectives were met by state and local agencies.
Remaining corrected ARCAs in Table 2 are identified by an asterisk.
Uncorrected ARCAs are subjects for inclusion in the next biennial exercise.
NOTE: New FEMA Objectives are listed in parenthesis.
