SUBJECT:

REPORT NOS. 50-327/88-43 AND 50-328/88-43 Thank you for your response of January 3,1989, to our Notice of Violation issued on November 1,1988, concerning activities conducted at your Sequoyah facility.

The controls over the unreviewed safety question determination (USQD) assumptions in your response dated January 3,1989, do not address the issue at hand.

Specifically, the USQD review process and the DCN approval process can be l

performed by different groups in a parallel effort.

For the example cited, the inspector held discussions with the parties who performed the USQD as well as j

the DCN and it appeared that the two efforts were not as well coordinated as your response indicated.

At the time of the inspection, the Sequoyah Reactor Engineer agreed that although you were fortuitous to be covered by adequate procedural controls in the specific example cited, it was not apparent that these controls would have been in place as a result of the DCN or USQD process.

Furthermore, subsequent to receipt of your letter, we reviewed current revisions of NEP 6.6 titled 10 CFR 50.59 Safety Evaluations, NEP 6.2 titled Design Change Notice, NEP 6.3 titled Operating Plant Modifications, and NEP 6.4 i

titled Plant Modifications Packages and could not find any specific controls to 1

ensure that the basis for the USQD or modification package is properly bounced.

Therefore, you are requested to provide a supplemental response within 30 days

of the date of this letter that will clearly indicate your controls to ensure l

that the USQD and design change properly address all aspects of the plant i

operational control process.
