Gentlemen:
This refers to the routine safety inspection conducted by Ms. Peggy R. Rescheske
of this office on January 9 through February 14, 1989, of activities at the
Clinton Power Station, authorized by NRC Operating License No. NPF-62, and to
the discussion of our findings with Mr. J. S. Perry and others of your staff
at the ccaclusion of the inspection.
The inspection focused on your refueling activities and included review of the
significant events which occurred during the first refueling outage at the
Clinton plant. The enclosed copy of our inspection report identifies the
areas examined during the inspection. Within these areas, the inspection
consisted of a selective examination of documents, observations, and
interviews with personnel.
During the inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Notice. A written response .
is raquired. On February 1,1989, a Confirmatory Action Letter (CAL-R111-89-005)
was issued as a result of significant refueling problems. The actions required
by the CAL should be considered separate from your response to the Notice
enclosed.
A number of significant events occurred during the course of the refueling
activities. .Two of the events were reportable as LERs. The causes of the
individual problems appeared to be related to one or more of the following:

ineffective and/or lack of training, inadequate communications, and
inattention to detail.
Your post-event corrective actions were adequate to
prevent recurrence of the specific event; however, based on our evaluation
of the problems and discussions with your staff, we believe that a lack of
effective administrative / management controls was common throughout the
events.
Your written response to the enclosed Notice should address our
,
concern regarding lack of effective management controls.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter, the enclosures, and your response to this letter will be placed
in the NRC Public Document Room.
