-Technical Specification (TS) 6.8.1.e' states that written procedures shall be established, implemented and maintained covering site Radiological Emergency Plan.(REP) implementation.
The Sequoyah Radiological Emergency Plan IP-1,. Emergency Plan Classification Logic, which implements these -
requirements, required that the operators enter a Notification of Unusual
' Event (NOUE) if;the primary. system leak rate was greater than that allowed in the TSs.
In addition, TS 3.4.6.2 states that reactor coolant system leakage shall be limited to 1 gpm unidentified leakage.
Unidentified leakage is defined-as all leakage which is not identified leakage or controlled leakage.
Contrary to the above, on April 6,1988, the licensee failed to enter a NOVE when RCS leakage. exceeded TS requirements as indicated by RCS leakage data obtained from the performance of SI-137.2, Reactor Coolant System Data Inventory.
