AIT CHARTER (DRAFT)

Enclosed for your implementation is the draft Charter for the inspection of the events associated with the LaSalle Unit 2 System Auxiliary Transformer (SAT) failure and Unit I reactor trip which occurred on March 2,1989. This Charter is prepared in accordance with the NRC Inspection Manual Chapter 0325.

The objectives of the AIT are to communicate the facts surrounding this event to regional and headquarters management, to identify and communicate any generic safety concerns related to this event to regional and headquarters management, and to document the findings and conclusions of the onsite inspection.

If you have any questions regarding thes'e objectives or the enclosed Charter,

9 DRAFT AUGMENTED INSPECTION TEAM (AIT) CHARTER LASALLE REACTOR SCRAM AND UNUSUAL EVENT 1 FOR PARTIAL LOSS OF OFFSITE POWER ON MARCH 3, 1989

)

You and your team are to perform an inspection to accomplish the following:

1. Develop and validate the sequence of events associated with the March 2 scram of LaSalle Unit 1 and the failure of the Unit 2 System Auxiliary l

Transformer (SAT). Include plant conditions immediately prior to the initiating events and any contributory causal factors leading to initiation of the event.

l 2. Review the cause of the loss of the Unit 2 system auxiliary transformer 1

(SAT).

3. Review why the loss of the Unit 2 SAT caused a turbine generator lockout signal or. Unit 1.
4. Determine which equipment responded abnormally, and why, including but not limited to:
a. Units 1 and 2 reactor building ventilation systems.
b. Unit 2 reactor water cleanup system.

5. Determine the cause of the water level fluctuations (between 25" and i 1

53.5") in the Unit 2 reactor including the lockup of the reactor i feedpump.

6. If necessary, develop a quarantined equipment list (QEL), and monitor i

the status of the equipment thereon.

7. Determine what equipment was activated by the power surge or voltage j spike caused by the Unit 1 main power transformer.
8. Evaluate the adequacy of the licensee's examination of plant equipment i and investigations to determine what additional equipment may have been i affected by the voltage transients.

9. Interview management to determine:
a. When it was first informed of the event.
b. The nature of the information communicated.
10. Evaluate the accuracy, timeliness, and effectiveness with which information on this event was reported to the NRC, including the Resident Staff.
11. Determine the adequacy of the licensee's plans to assure no further degradation of the Unit 2 station power in the event that a shutdown is required; i.e., the loss of cffsite power while in the unusual event.
12. Determine the adequacy of the licensee's "new" procedure for a controlled shutdown if unable to return the SAT to service; i.c., a loss of offsite power will occur under controlled conditions.
13. Evaluate the findings and identify those for which generic communication may be applicable. Provide recommendations to regional management.

This letter confirms the telephone conversation between J. J. Harrison of this office and Mr. G. J. Diederich of your ' staff an March 5,1989,  ;

regarding a Confirmatory Action Letter (CAL-RIII-89-007) for LaSalle Units 1 and 2. This CAL was verbally issued on March 3, 1989, and formally a issued on March 6,1989, regarding the Unit I reactor trip and loss of I the Unit 2 System Auxiliary Transformer (SAT) that occurred on March 2, 1989. On March 3, 1989, we established an Augmented Inspection Team. 1 (AIT) to independently assess the cause and significance of the event and any associated problems.

On March 4,1989, the AIT completed its assessment and_ conducted its exit meeting with the LaSalle station staff. The AIT determined-that CECO had adequately evaluated the event and satisfied the actions required by the CAL. Therefore, .on March 5,1989, I concurred with your decision to .j restart Unit 1 and we consider items l'through 5 of CAL-RIII-89-007 closed. l We await your formal report as required by _ item 6.of the CAL.
