SUBJECT:

CONFIRMATION OF ACTION LETTER This is to confirm the conversation between Mr. A. B. Scott of your staff and Mr. R. F. Warnick of my staff on May 5, 1989, regarding the incident of backleakage through the Borg-Warner check valves in the Auxiliary and Main Feedwater Systems on April 23, 1989 during the Unit 1 Hot Functional Testing.

Based on our conversation, it is my understanding that TU Electric is taking the following actions:

1. All records and instrumentation records relevant to the event will be gathered and preserved by TV Electric.
2. Additional testing will be perfomed to identify the individual feed system check valves that are leaking.

Thermal stress analysis will be performed and results made available for review.

3. After the Hot Functional Test is ccmpleted and the plant cooled down, check valves with confirmed leakage and those with similar operational characteristics will be opened for inspection and corrective action taken. The NRC site inspection team (H. Livemore) is to be notified of the date and time each check valve is to be opened so that the NRC can be present and observe the work in-process and the condition of the valve.
4. Valve internals and other evidence that might be important to the identification of cause of the check valve backleakage will be entered on the Quarantined Equipment List (QEL) and preserved until released by the NRC.
5. The NRC site inspection team will be kept apprised of current activities, findings, and plans relating to the check valve backleakage problem as well as all plans for reinstallation and retest.
