Dear Mr. MacDonald:
We request renewal of the USNRC Certificate of Compliance Number 9033 issued for the Model 660 exposure device.
We' request that the following revisions be incorporated in the renewed certificate.
We have modified the labels applied to the 660.
These are coscribed in crawing numbers 66001-19, 66001-18, 66001-9 and 66001-10.
These contain all the required _information and will be legible after a fire test.
l We also request that the 660 be authorized to transport the I
following additional isotopes in approved source assemblies.
Each of these has been approved as special form radioactive material and has been registered for use in the Model 660.
