Subject: Millstone 2 Resident Inspection 50-336/89-05 (2/11/89 - 3/23/89)
The enclosed report refers to the above subject inspection.
The inspectors'
findings have been discussed with Messrs. Scace and Keenan of your staff.
Section 8.1 of the report describes a security event that was identified by
your staff and reported to the NRC in accordance with requirements.
This item
is currently under review by management for appropriate enforcement action.
Our conclusion regarding this matter will be transmitted in separate corres-

pondence.
Report Detail 11 and your April 3,1989 letter (813185) address the replacement
i
of in core detectors. We note that you plan, based on the absence of any poten-
tial safety impact, to submit a Technical Specification change to permit simi-
lar activities without senior reactor operator overvies or containment integ-
rity.
Literal adherence to Technical Specifications is required, and the Tech-
nical Specification change should be approved before implementing any changes.
Pending resolution of the change request, this matter is unresolved.
Report Detail 11 notes that Millstone Station ACP-QA-3.02 allows undocumented
minor deviations from procedures. Our preliminary evaluation is that, if such
deviations are not at least documented and dispositioned like other minor dis-
crepancies (e.g., unexpected results), the procedure has been violated. Our
safety review of such conditions at Millstone has identified no safety inade-
quacy.
Nonetheless, undocumented and unreviewed procedure discrepancies have
potential safety consequences, particularly since errors may be compounded over
time.
The associated compliance with TS 6.8.1 and your quality assurance prc'
gram (e.g., ANSI N18.7-1976 Section 5.1.2) will be reviewed further.
Your repair of potentially defective steam generator tube plugs is discussed in
