Subject: NRC Inspection of the Trojan Nuclear Plant

This refers to a special inspection conducted by Mr. R. C. Barr and Mr. J. F. Melfi

cf this4 office from July 12, 1989 through August 8,1989, of activities

authorized by NRC License No. NPF-1, and to the discussion of our findings

held by Mr. Barr and.Mr. Melfi with Mr. Cockfield and other members of your

staff periodically during the inspection.

Areas examined during this inspection are described in the enclosed inspection

report. Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with

personnel and observations by the inspectors.

Based on the~ results of this inspection, it appears that certain of.your

activities.were not conducted in full compliance with NRC requirements.

Specifically, the inspection determined.that debris in the containment

recirculation sump, as well as missing and damaged sump screens rendered the

sump inoperable. Further, this deficiency would likely have prevented both

trains of the Emergency Core Cooling System from functioning as required had

the equipment actually been called upon during the recirculation phase of a

loss of coolant accident. The safety significance of these conditions and the

high potential that a problem could occur, are best demonstrated by your own

experience in 1980, when a Residual Heat Removal pump was found to be Jamed

due-to a piece of weld rod between the impeller ring area and casing ring.

Our inspection also determined that your actions from many previous occasions,

when debris was found in the containment recirculation sump, were not adequate

to prevent recurrence. Furthermore, the containment recirculation sump condition

went undetected because your surveillance activities were not adequate to verify

sump cleanliness or selected design features. Additionally, there were several

other opportunities, e.g., Design Basis Document reconstitution and followup

on Generic Letter 85-22, for PGE personnel to find the problems and initiate

resolution. Your inability to learn from these previous opportunities causes

us serious concern.

Finally, our inspection findings indicate that PGE management oversight of

plant activities associated with the sump have been ineffective. Specifically,

management had not. assured that PGE personnel were aware of the safety

- significance of. the containment recirculation sump, and had not developed the

', appropriate sensitivity and conservative instincts needed to identify serious

challenges to safety-related equipment.

- PGE management should ensure prompt and effective action is taken to correct

the individual as well as programmatic deficiencies identified in this report.

These matters will be discussed with you during a management meeting to be

arranged in the near future. The apparent violations of NRC requirements

described in the enclosed report will be the subject of future correspondence. .
