PILGRIM STATUS REPORT FOR THE PERIOD APRIL 22 - MAY 5, 1989
- Enclosed is the Pilgrim bi-weekly status report from the NRC Resident Office at Pilgrim..The NRC Restart Staff is in place and was on-site monitoring licensee activities on an around-the-clock basis during plant operation at the 25% power level and on an. extended-shift basis during periods of' reduced plant activities..
The Pilgrim-bi-weekly-status report is intended to provide NRC management and the public with an overview of plant activities and NRC restart inspection activities. ' Subsequent inspection reports will address many of these topics in more detail.

1 ENCLOSURE Pilgrim Status Report for the Period April 22 - May 5, 1989 k

' 1. 0 Plant Status 1
As of 8:00 a.m. on May 5,1989, the reactor was in the cold shutdown mode with moderator temperature about 123 degrees F.
2.0 - Power Ascension Test Program Status The licensee has completed t_he original scope of the the Power Ascension Test I'rogram up to the 25?; power plateau.
Prior to continuation of the power ascension program to_the.50!s power plateau, the licensee must obtain the NRC Region I Regional Administrator's concurrence.
The program includes NRC approval points at 5*4, 25!!, 509; and 759s of full power and both a licensee written report and NRC review after completion of testing at full power.
The NRC Restart Assessment Panel will recommend to the Regional. Administrator when the plant is considered ready to be released from each approval point.
3.0 Facilities Operations Summary As reported in the last Pilgrim Status Report (April 8-21,1989), the plant was in cold shutdown at the beginning of this report period follow-ing the plant shutdown due to the unexpected opening of the Reactor Core Isolation Cooling (RCIC) system injection valves during a test on April 12, 1989.
At 7:45 p.m. on April 28, 1989, the licensee brought the reactor critical.
The turbine generator was synchronized to the electrical power distribu-tion grid at 7:28 a.m. on April 30, 1989.
At 3:25 a.m. on May 3, 1989, while licensee personnel were troubleshooting the "B" Feedwater Regulating Valve (FRV), the FRV opened unexpectedly, causing reactor water level to increase rapidly.
This resulted in a turbine trip due to high reactor vessel water level. The turbine trip caused the reactor to automatically shutdown (i.e.,
scram) cs designed.
All safety systems responded as designed and expected with the exception of five containment isolation valves which performed their safety function (i.e., went closed) unexpectedly. The licensee placed the reactor in cold shutdown on May 3,1989.
The licensee has formed a multi-disciplinary investigation team to perform evaluation and troubleshooting of this event. Detailed NRC review of the above event and licensee actions will be documented in Inspection Report No. 50-293/89-06.

The plant remained in the cold ' shutdown mode throughout the remainder'of the report period while the licensee continued reviewing the scram and troubleshooting the isolation valve closures and FRV problem. The licen-see continued to conduct routine surveillance along with preventive and corrective maintenance.
4.0 Items of Special Interest Oral Arguments on Commonwealth of Massachusetts 3 Petition to Shutdown Pilgrim Station On December 21, 1988, the Attorney General (AG) of the Commonwealth.of-Massachusetts filed a petition with the U.S.
Court of-Appeals for the.
First Circuit requesting the court to set aside the Commission's. decision to allow Pilgrim Station to restart. The stated grounds of the petition were the incomplete status
'of offsite emergency plans.
On February 21, 1989, the AG filed a motion requesting the Court to ' stay tN NRC from permitting Pilgrim to operate above 5% of full power.

On March 2, 1989, the staff responded to the February 21, 1989 motion filed by the AG. The AG's motion for a stay was subsequently denied by the Court on March 6, 1989. During this ' report period, the U.S. Court of Appeals for the First Circuit began ht.aring oral arguments on the petition filed by the AG for the Commonwealth of Massachusetts.
Public Demonstration Approximately 70 people held a demonstration outside the Pilgrim plent on April 30, 1989.
Three people were arrested for trespassing.
5.0 Emergency Notification System (ENS) Report On May 3, 1989, the licensee experienced a high reactor vessel water level. The event was reported to NRC by ENS and is described in detail in Section 3.0 above.
6.0 Pilgrim Restart Staff Activities The Pilgrim restart staff began around-the-clock shift coverage at 6:30 a.m. on April 24, 1989. This coverage was reduced to extended shift coverage at midnight on May 3, 1989 consistent with reduced testing activity. Additional personnel from NRC Region I, the Office of Nuclear Ractor Regulation (NRR) and an NRC contractor were onsite during the report period to augment the resident inspection staff.

A NRC-developed inspection n addressing the 5-25% power plateau and the primary functional areas
- perations, maintenance, surveillance, and startup testing continued so be implemented.
The inspectors assessed operator performance, organizational interfaces, adherence to procedures and technical specification compliance.
The Pilgrim Restart Staff continued to evaluate the performance of licen-see personnel and the plant to determine whether the licensee should be permitted to proceed to the next NRC approval point, a maximum of 50%
rated power. The Restart Staff and the Pilgrim Restart Assessment Panel will review the April 12, 1989 RCIC system event and May 3,1989 turbine trip / reactor trip event before finalizing that determination.
Contact with the press and public continued throughout the period.
