APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket:
50-458 River Bend Station Operating License:
NPF-47 During an NRC inspection conducted during March 27-31, 1989, a violation of NRC
. requirements was identified. The violation involved failure to impose 10 CFR Part 21 on a vendor providing basic components.
In accordance with the-
"Gemal Statement of Policy and Procedure for NRC Enforcement Actions " 10 CFR Part 2,' Appendix C. (1988), the violation 'is listed below:
' Section 21.31 of 10 CFR Part 21 requires the licensee to specify on each procurement document for a basic component that the provisions of 10 CFR Part'21 apply.
Contrary t'o the above, Purchase Order 8-10-72689 was placed with the Okonite Company for the supply of a basic component without specifying that;the provisions of 10 CFR Part 21 were applicable.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office, within 30 days of the date of the letter transmitting this' Notice, a written statement or explanation in reply, including for each violation:.(1) the reason for the violation if admitted, 2)thecorrectivestepswhichhavebeentskenandtheresultsachieved,
- 3) the corrective steps which will be taken to avoid further violations, and
- 4) the date when full compliance will be achieved. Where good.cause is shown, consideration will be given to extending'the response time.
