SPECIAL REPORT I

INOPERABILITY OF CHANNEL No. 6 OF THE STEAM GENERATOR IB

)

l LOOSE PARTS MONITOR SYSTEM 1

i On May 21, 1989 Channel No. 6 of the Catawba Nuclear Station Unit 1 Steam Generator IB Vibration and Loose Parts Monitor System was declared inoperable.

The inoperability of Channel No. 6 was discovered by Operations personnel when i

the Channel emitted c 60 cycle signal after being selected for indication.

{

Previous malfunctions of this system at Catawba Nuclear Station involved loose 4

wiring, loose connections, or faulty accelerometers.

Channel No. 10 of the Unit 1 Steam Generator 1A is operable. Therefore, a redundant channel is available. Work request 506860PS was written to investigate and repair the channel. The repair will be ca:.ied out during a future outage of sufficient duration. This work is to be performed during an outage due to Channel No. 6 being located in the steam generator cavity. This cavity is inaccessible during power operation because of radiological considerations.

The inoperability of Channel No. 6 for more than 30 days is reportable pursuant to Catawba Technical Specification 3.3.3.9.
