ENFORCEMENT CONFERENCE
SUMMARY
This letter refers to the Enforcement Conference held at our request on May 24, 1989.
This meeting concerned activities authorized for your North Anna facility. The issues discussed at this conference related to the service water system heat transfer capability and reactor inventory perturbations. A list of attendees, a summary, and a copy of your handout are enclosed.
We are continuing our review of these issues to determine the appropriate enforcement action.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and its enclosures will be placed in the NRC Public Document Room.

ENCLOSURE 2 ENFORCEMENT CONFERENCE
SUMMARY
An Enforcement Conference was held at the NRC's Region II Office with Virginia Electric and Power Company (VEPC0) on May 24, 1989, to discuss issues concerning North Anna's service water (SW) system and reactor vessel inventory events.
VEPC0's Senior Vice President,. Nuclear, opened the meeting by stating i
.that the enforcement issues have'been thoroughly reviewed and followed up. The North Anna Station Manager provided a management perspective, and stated that the two enforcement issues were handled separately because the root causes were different.
For example, VEPC0 determined the root cause of the SW heat l
transfer issues to be a configuration management problem and the root causes for the reactor. vessel inventory events to be procedural inadequacies and lack
'i of management sensitivity.
.The Assistant Station Manaaer began the formal presentation, using the handout detailed in Enclosure 3.
Design basis, system description, sequence of events, and root cause/ corrective actions were discussed for the issues related to SW.
Two dates were corrected on the handout, as annotated in Enclosure 3.
With regard to the safety significance of the SW issues, the Assistant Station Manager stated that analyses showed that if only two recirculation spray heat exchangers were flowed with SW during tests, the results would have exceedei the design basis flow.
The Senior Resident Inspector asked if the analyses 4
accounted for SW diverted to the component cooling water system, and VEPC0 responded that they would investigate and inform the Senior Resident Inspector.
l The Station Manager concluded this part of the presentation by stating that the

Merits Program and Configuration Management Program will assist in identifying and reviewing similar incidents in a systematic manner.
l VEPCO's presentation then focused on the reactor vessel inventory events. The sequence of events was extensively discussed as outlined in Enclosure 3.
The licensee provided a self-assessment of the events and concluded that the root causes were inadequate procedure and lack of supervisory sensitivity after the first event occurred.
The licensee stated that a permanent reactor coolant I
system level indicator will be installed.
J The Vice President, Nuclear Operations, concluded the presentation by stating j
that VEPC0 conducted a good analysis of the facts, root cause, and significance 1
of each issue.
The NRC Regional Administrator closed the meeting by thanking VEPC0 for the presentation.
