A'PPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-352 Limerick Unit I License No. NPF39 j

As a result of an inspection conducted on April 23-May 28, 1989, and in accordance with NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

A.

Technical Specification 6.8.1 requires written procedures to be established, implemented and maintained to control various plant activities.

1.

Operating Procedure S47.8C Step 8.2.2 refers to procedure RT-11-00427-for removal of a Hydraulic Control Unit (HCU) and step 6.1 of that procedure says " ensure a temporary circuit alteration (TCA) has been initiated for the associated control rod".

Contrary to the above, during the removal of HCU 58-43 on May 15, 1989 the operator did not initiate a TCA to remove the HCU from service.

2.

Administrative procedure A-41 requires that the individual have the blocking permit or a facsimile in hand to remove blocking tags.

Contrary to the above, during the unblocking of HCU's, the operators did not have a blocking permit or facsimile and subsequently, on May 6,1989, the operators failed to remove the blocking tags on HCU 14-43.

Surveillance Test Procedu: e ST-2-049-614-1 RCIC requires that test equipment be removed prior to keylock switch B21B-55C being returned to normal.

Contrary to the above, on May 28, 1989, the I&C Technician turned the keylock switch B218-55C to normal prior the removal of test equipment causing a RCIC isolation.

These three examples constitute a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office '.dthin thirty days of the date of the l

letter which transmitted this Notice, a written 'tatement or explanation in l

s reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further 1

violations; and (3) the date when full compliance will be achieved. Where good I

cause is shown, consideration will be given to extending this response time.

APPENDIX B NOTICE OF VIOLATION Philadelphia Electric Company Decket No.

50-353 Limerick Unit 2 License No. CPPR-107 As a result of the NRC Independent Design and Construction Assessment corrective action inspection conducted at Limerick Generating Station, Unit 2 during the period of March 27-31, 1989, and in accordance with 10 CFR 2, Appendix C the following violation was identified:

10 CFR 50, Appendix B, Criterion X requires that inspections be conducted to assure conformance of work activities with appropriate design documents.

The Philadelphia Electric Company Quality Assurance Plan Volume 1, Section 10, states that inspections are performed in accordance with inspection criteria derived from appropriate documents to assure the quality of installation activities.

Contrary to the above, inspections were not conducted to assure conformance of work activities with appropriate design documents.

1.

Jam nuts or staked threads were not provided on foundation bolts for the standby liquid control pumps in accordance with Bechtel drawing C-631.

2.

Washers were not provided on the residual heat removal system heat exchanger 2A foundation bolts in accordance with Bechtel drawing C-615.

3.

The residual heat removal pump shaft seal assembly drain ports were

sealed with plastic shipping plugs in-lieu of the design specified j

steel plugs.

4.

The existence of bolt clearance to accommodate thermal growth of the

{

residual heat removal heat exchanger lower support in accordance with General Electric Specification 21A9227AT could not be verified.

Washers were not installed on the anchorage bolts for unit cooler 2FV211 as depicted on vendor drawing 107D-153890-G.

A high strength bolt on structural platform 55 was installed with the threads included in the shear plane, which was contrary to i

Bechtel drawing C-1184.

The above examples constituted a Severity Level IV violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within 30 days of the date of this letter transmitting this Notice, a written statement of explanation in reply to the violation to include:

(1) the corrective actions which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response required time.
