CONFIRMATORY ACTION LETTER NO. CAL-RIII-89-016 Gentlemen:

On June.1, 1989, the Clinton Power Station "B" recirculation (RR) pump seals.-

failed concurrent'with complications involving the drywell chillers and the motor driven' feedwater pump regulating valve tFRV).

Confirmatory Action. Letter (CAL-RIII-89-016) was issued as a result of the

' June l. 1989 event and required the following licensee actions:

. Perform an evaluation as to the cause of the failure of both seals on the RR pump,'the inability.of the FRV to control level and the trip of'the drywell chiller.. Any systems or components which did not function properly will be included.

Identify any equipment that malfunctioned and place such equi) ment into a quarantine status until the NRC Special Inspection Team has t1e

' opportunity to review yow action plan regarding this equipment. This would include any equipment so designated by the Team Leader (P. Hiland).

The RR pump seals and the FRV would be expected to be included in the quarantine. Removal of the seal aackage from the RR pump is authorized provided that no disassembly of t1e seal package is performed.

3.

Evaluate the equipment history of those components believed to have malfunctioned and include in the quarantine any previously replaced

components as are still available.

4.

Except as dictated by plant safety, advise the NRC special team leader prior to conducting any troubleshooting activities. Such notification should be provided soon enough to allow time for the team leader to assign an inspector to observe activities.

.5.

Submit to NRC Region III a formal report of your findngs and conclusions within 30 aays of receipt of this letter.

The Confirmatory Action Letter also required the concurrence of the Region III Regional Administrator or his designee prior to the startup of the Clinton l.

plant.

On June 16, 1989, the' Regional Administrator was informed of the status of the l'

investigations'into the failure of the RR pump seals, the FRV and.the trip of the operating drywell chiller which occurred on June 1,1989. The corrective.

l actions initiated.by the licensee and the precautionary measures instituted by the licensee such as increased monitoring of RR pump parameters were adequate l

.to' justify a restart of the plant. Accordingly, the Regional Administrator b

authorized restart and power operation activities on June 16, 1989 as reflected in the June 21, 1989 Region III letter.

Illinois Power (IP) Company letter dated June 30, 1989, provided the final written report required by CAL-RIII-89-016. The report detailed IP's findings and conclusions of the investigation into the RR pump seal failure and the concurrent failure of the FRV and the trip of the operating drywell chiller A.

~ All actions taken to satisfy CAL-RIII-89-016 action items 1 through 4 were described in the report. The Region III staff has reviewed IP's findings and conclusions and'has determined that IP's corrective actions to address the investigation. findings were adequate.

i-We consider all CAL-RIII-89-016 Action Items appropriately resolved.

The corrective actions initiated as a result of the CAL and IP's findings should' reasonably preclude similar future problems at Clinton Power Station.

Therefore, I hereby terminate this Confirmatory Action Letter.
