'I. Statement of Violation -

STP Station Proceduro OPGP03-ZS-0001,. Revision 5. " Personnel

Access Control," Addendum 2, requires, in part, that visitors

display their visitor badge'on the front of their body between the

shoulder and the waist.

Contrary to the above, on March 6, 1989, during a tour of the STP,

Unit.2, Fuel Handling Building (FHB) a visitor was found without

his badge attached to his body between the shoulder and waist.

His badge was attached to a jacket which was laying on the floor

on a different elevation on the FHB.

II. Houston Lighting & Power Position

HL&P concurs that the cited violation occurred.

III. Reason for Violation

The root cause of this violation was human error in that the ,

visitor and his escort failed to follow established security j

procedures. j

IV. Correcrive Action Taken and Results Achieved

The following corrective actions were taken immediately upon

security receiving notification of the unbadged visitor: 1

1. The number of the misplaced visitor badge was immediately

entered into the " Lost" file of the security computer.

(Note: The STPEGS security system prevents a visitor keycard

from accessing a cardreader-controlled door without the

keycard of the assigned escort.)

2. Preparation of Individual Access Transaction (IAT) Reports was

initiated to verify that no unauthorized keycard usage had

occurred.

,

3. The visitor and his escort were placed under security control

and escorted to the East Gatehouse for the purpose of

verifying and documenting details of the incident.

IAT reports initiated at 1023 and 1101 hours0.0127 days <br />0.306 hours <br />0.00182 weeks <br />4.189305e-4 months <br /> verified that there

( had been no unauthorized use of the subject visitor badge.

l' V. Corrective Actions Taken to Prevent Recurrence

The following actions will be taken to prevent recurrence:

1. Security personnel processing visitors have been instructed to

emphasize the requirements printed on the reverse side of the j

Visitor Access Request Form to the visitor prior to signature

'

by the visitor.

2. Break-away chains to enable wearing badges around the neck i

have been placed at each visitor registration point and will

be issued to visitors entering the protected area.

VI. Date of Full Compliance
