Gentlemen:

This refers to the special safety inspection conducted by .

Mr. J. McCormick-Barger of this office on June 5-9, 1989, of activities at

Davis-Besse Nuclear Power Station authorized by Facility Operating License

No. NPF-3 and to the discussion of our findings with Mr. Ramsett and others

of your staff at the conclusion of the inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

No violations of NRC requirements were identified during the course of this

inspection.

The NRC has concerns about your use of Request for Information (RFI) forms and

Document Change . Requests (DCRs) to correct deficiencies identified in design

documents. Please respond to this concern within 30 days of receipt of this

letter. Your response should include a discussion of how your current program

determines design document deficiency trending, and when pervasive problems

are found to exist, how root cause analyses and additional reviews and actions

are performed to assure adequate resolution.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy

of this letter and the enclosed inspection report will be placed in the

NRC Public Document Room.
