Confirmatory Action Letter CAL-RIII-89-016 were discussed. Our understanding i
of the discussion /comitments from that meeting is as follows:

IP had' performed a thorough investigation into the failure of both seals on the B recirculation (RR) pump,'but was unable to conclusively.
j detemine the root.cause of the failure, partly.due to the destruction

Installation error was believed to.be the i
mostly likely root cause although little evidence was available to confirm any of the possible causes. Because of this situation, IP planned
.to provide for increased monitoring of the RR pumps, particularly-the B pump, during-plant startup. This monitoring was expected to consist of additional engineering personnel on-shift to evaluate pump parameters; increased observation of the installed instrumentation including the seal pressures, seal temperatures, pump motor vibration, seal injection flow, and seal leakage; and hand-held vibration readings.when the pump was energized, at 500 psig, and at nomal operating pressure.
Further, we understood that IP would keep the NRC infomed of any anomalies in RR pump seal behavior during startup.

IP.had performed an extensive investigation into the inability of the motor driven feedwater pump regulating valve (FRV) to control level.
The major contributor to the FRV problems was believed to be vibration and its effect on the control circuitry which was mounted on the valve operator. The' junction box containing this circuitry was moved from the valve operator and mounted on the wall so it would be less susceptible to vibration. The. final fix to the FRV problems may involve a major design change to the piping configuration and the FRV itself; however, in order to startup IP plans to closely monitor the performance of the FRV. This FRV is only used on startups and shutdowns, and operations on this FRV are only expected to last less than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

IP had performed an investigation into the trip of the running drywell chiller during the seal failure event. Root cause of the trip was believed to be the momentary decrease in service water pressure at the chiller inlet due to the increased head load caused by the event. This pressure fluctuation coincident with a high setpoint on the low pressure trip is believed to have caused the trip.-
IP has performed various.
actions to prevent recurrence, including lowering the the trip setpoint and installing a pulsation dampener in the pressure sensor line to dampen out the. initial pressure decrease spike.
Based on the above discussions, I hereby authorize you to startup and operate the Clinton plant as verbally communicated to Mr. D. Hall, Senior Vice President, Illinois Power, on June 16, 1989.
