We have reviewed your response dated June 16, 1989, regarding four allegations that we forwarded to you by our letter dated May 3,- 1989. Our letter requested that you submit the results of your investigations into:
1.
Upgrading unqualified auxiliary operators ("C" men) to equipment attendants ("B" men) that were not accompanied by a foreman while on rounds which included safety related tasks, between December 25, 1988 and January 1, 1989.
2.
During the end of the 1988 Unit 2 outage, the overtime guidelines of Zion Administrative Procedure, ZAP-0 were violated by at least one NSO working 20 consecutive hours (ZAP-0 guideline is 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> in a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> period).

Service water flow meters for the ESF pump room coolers had been abandoned and outstanding work requests were cancelled resulting in log readings which are meaningless.
4.
Open electrical conduits in ESF pump rooms, which communicate across fire barriers and no corrective actions had been taken to resolve the problem.

We request that the results of your review and disposition of these matters be submitted to Region III within 30 days of the date of this letter. We i
request that your response' contain no personal privacy, proprietary, or safeguards information so it can be released to the public and placed in the NRC Public Document Room.. If necessary, such information shall be contained in a separate attachment which will be withheld from public disclosure and the affidavit required by 10 CFR 2.790(b) must accompany your response.
The enclosure to this letter should be controlled and distribution limited to personnel with a "need to know" until your investigation of the allegation has been completed and reviewed by NRC Region III. The enclosures to this letter are considered as Exempt from Public Disclosure in accordance with Title 10, i
Code of Federal Regulations, Part 2.790(a). However, in accordance with.

Allegation:
I 1.
That from December 25, 1988 through January 1,1989, personnel upgrades were made from auxiliary operators to equipment attendants, unqualified upgraded individuals were not accompanied on rounds by a foreman, and this action resulted in safety related tasks being performed by unqualified individuals.

That the ESF pump room coolers, cooled by service water, have flow meters which have been abandoned and outstanding work requests cancelled resulting in log readings which are meaningless.

That in the ESF pump rooms, open electrical conduits exist which I
communicate across the fire barriers and no corrective actions have been taken to resolve this problem.
We believe that to adequately resolve this issue, the following should be addressed:
a.
As related to concern 1, determine whether auxiliary operators ("B" men) were upgraded to equipment attendants ("A" men) during the December 25, 1988 thrc> ugh January 1,1989 period; and if unqualified "A" men performed safety related tasks without supervision by an accompanying shift foreman.
Contact "B" men upgraded to "A" men and shift foremen responsible for supervising the rounds. Review logs, shift manning charts, Shift Engineer's logs and training records.
b.
As related to concern 2, determine whether actual hours worked by operators and key maintenance personnel met the limits of ZAP-0 overtime guidelines and the NRC limits of Generic Letter (GL) 82-12. Review payroll records for affected personnel during this period and include Shift Engineer's logs from October through December 1988.
c.
As related to concern 3, determine the minimum Service Water (SW) flow needed to assure that the ESF pump room temperatures remain within an acceptable range; and the actual flow rates through the ESF pump room coolers. Review the design basis statements and/or calculations, SW flow meter calibration data and logs of SW flow readings to the ESF pump room coolers for the period of 1988.

As related to concern 4, determine whether open electrical conduits exist in the ESF pump room, whether any open conduits found render the associated fire barriers degraded or inoperable and what, if any, corrective action was taken for open conduits found during the period of January 1 through 31, 1989.
These items are forwarded as an indication of the minimum effort that would resolve this issue but are not intended to limit or prevent your pursuit of other additional measures you determ?ne are appropriate for addressing the allegation.
