l NOTICE OF VIOLATION

Wisconsin Electric Company Docket Nos.: 50-266 1 50-301 j 1

As a result of the inspection conducted from March 1 through May 31, 1989, and i in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1987), the following violation was identified:

Technical Specification 15.6.8.1 requires that the plant shall be operated and maintained in accordance with approved procedures.

a. Administrative Procedure 4.13, Equipment Isolation Procedure, states that red tags may be only installed, verified,'or removed by plant persennel who have been qualified to do so by the Superir.tendent - Operations and whose names appear on the " Qualified Red Taggers" list. This list is maintained by an Operations group Standing Order (PBHP 4.12.2).

Contrary to the above, on May 1,1989 the installation, verification, and removal of red tags by plant personnel was being controlled through the use of a modified " Qualified Red Taggers" list attached to the active red tag log instead of the list contained in Standing Order 4.12.2.

b. Instrumentation and Controls Procedure (ICP) 11.422, Initial Checkout and Test of AMSAC, MR 85-213, stated that if steam generator levels are expected to go below the low level setpoint, install temporary analog simulators in the reactor protection racks and simulate signals of approximately 50% for two steam generator levels in each loop.

Contrary to the above, on May 5,1989, the technician failed to install the analog simulator for the 1A steam generator which was scheduled to be drained below the low level setpoint to facilitate maintenance.
