This refers to the routine safety inspection conducted by site inspectors from
April 22 to June 2, 1989, of activities authorized by NRC License No. DPR-73
and to discussions of our findings held by Mr. T. Moslak of our office with you
and other_ members of your. staff on' June 5,1989.

During ~ this inspection period, information was provided by 'your organization to

the NRC. staff regarding a transfer of a core debris sample without the formal'
l
notification to senior ' licensed personnel 'ursuant to TMI-2 Technical Spect-
l
p
fications.
This breakdown in command and control is regarded as a communica-'
.j
tion failure of supervisory personnel. Although we have no'immediate safety
'
concerns, this is an area where increased management attention may be war--
ranted. Because of your prompt self-identification of the incident, timely.
response and management's immediate involvement, a violation ~will not be cited
because the criteria specified in Section V.G. of the Enforcement. Policy of 10 CFR 2 are satisfied.
