Gentlemen:
' This provides'our understanding'of the meeting held in our ' offices October 21, 1987 to discuss four occurrences during your current refueling outage.
It also outlines our understanding of three commitments, which we understood you to make regarding engineering assessments to be conducted.
The list of attendees at the meeting is attached.
The four events which were discussed were:
a resin spill.
an apparent hydrogen burn in the pressurizer a fatality by electrocution an ESF actuation which resulted in lake water being pumped into the feedwater system and steam generators-Copies of the handout material you provided are attached.
We understood that you have commited to complete engineering evaluation to determine the effects of the apparent hydrogen burn in the pressurizer, the potential for damage to batteries from the deep discharge, and the effects on the feedwater system and steam generators from the introduction of lake water. We also understand that you will coordinate 'these engineering studies with the NSSS or other vendors as appropriate.
Your letter of October 20,1987(serialWM 87-0277), which outlined your resumption of outage activities, was discussed.

Suspension of Refueling Outage Wo 87-0301 As you all know, we have tmporarily suspended the refueling outage. In this memo, I will discuss why, and what we need to do in order to resume work.
For the most part, this outage has gone very well. The planning and prepare, tion of work activities has been evident in their performance. In the last week, however, a man has lost his life, and we have had three other incidents where potentially serious mistakes have been made.
Individually, these three may not have warranted drastic action. Taken together with the fatality, they are cause for alarm.
We have suspended the outage to allow us time to complete our investigations of these events, evaluate the root causes, and consider actions necessary prior to resunption of work.
It will also give us a chance to reflect on our philosophy and work practices, and give everyone a rest. Therefore, while our investigations and evaluations are being performed, we have asked all organizations to meet individually ard review key elements of our program. These include the careful planning of our work, taking the time to do it right, ard attention to detail. These elements apply equally to management and employee.
With the completion of our investigations and any corrective actions, we will initiate the resumption of outage activities. With the continued dedication and conscientious effort of all of us, we will complete thu' outage in a safe and ef ficient manner, ard resune power operation.

1 l., Each individual has a responsibility to assure their own safety. Never begin a job if you are unsure of your ability to do it safely.

Each individual should be knowledgeable of the established Work-Hour Limitations that are required for all personnel. Although there are exceptions, these should be kept to a minimum and approved through the i
proper supervision.
Keep in mind that a schedule is a target, a l
sequence of events which helps to coordinate the many activities that must occur. This is what enables efficient work and tim savings.

Each job should receive the necessary Attention to Detail that not only l
achieves the end result, but ensures that the proper Job Briefing has l
occurred, proper Supervision of the Job is maintained, and the activity is Done right the first time.
4.
In all work activities, Comply with the procedure. If the procedure doesn't work, is cumbersome, or is just too confusing: have it revised before continuing the job.
We cannot undo the loss we have suffered or change the events of the past.

Memo to Supervisors:
Pre-Job Briefing Pre-Job Briefings have always been a good idea and most Jobs used it as a tool for a better performance. They were not a requirement placed upon the work force by management. The good practice will now be required. A Pre-Job Briefing for all work at Wolf Creek will take place as a specific function for all work activities, in or out of an outage.
It will take place between the worker (s) and the lead or supervisor, prior to commencing work.
It will be as short-or long as the complexities of the Job require, but a few specific areas will be covered in each briefing. These will include:
- 1. Safety precautions for the Job including the clearance order.

- 2. The actual procedures to be used will be reviewed.
- 3. A general discussion of the Job complexities and potential problems.
- 4. This function, a pre-Job briefing, will repeat prior to each shift starting work in the field if a Job is an on-going one.
This concept must become a way of life at Wolf Creek.
It is an appropriate level of control in our business.
