SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT NOS. 50-338/89-03 AND 50-339/89-03)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
J. L. Caldwell on February 3 - March 20,1989.
The inspection included a
review of activities authorized for your North Anna Power Staion.
At the
conclusion of the inspection, the findings were discussed with those members of
your' staff identified in the enclosed Inspection Report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violations, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violation.
Violation A of the enclosed Notice of Violation concerns your
failure to take adequate corrective actions to deficiencies previously
identified in the condenser air ejector radiation monitor prior to the 1987
North Anna tube rupture event.
Your response to the violation associated with
the 1987 erratic behavior of the condenser air ejector monitor stated that the
source of the monitor's problems was moistse in the monitor lines.
During
the March 1989 tube leak incident, the condenser air ejector monitor once
again responded erratically.
In your response to the enclosed violation,
please provide information addressing how your actions will be more effective
than those previously taken to resolve problems with the condenser air ejector
[
monitor.
In addition, we continue to be concerned with problems related to your drawing
control program, as discussed in paragraphs 5, 7 and 10 of the enclosed report.
Accuracy of station drawings is of extreme importance. Based on a conversation
on April 14, 1989 between M. Bowling of your staff and P. Fredrickson of my
staff, we understand that you are consolidating your actions with respect to the
drawing control problems and will submit your current corrective action effort
to us by April 30, 1989, as a supplement to your response to Inspection Report
338,339/88-31.
Your attention is invited to the unresolved item identified in the Inspection
Report. This matter will be pursued during future inspections.
