Gentlemen:
Pursuant to 10 CFR 50.54(f) the following is Toledo Edison's response to NRC Bulletin Number 89-01, Failure of Westinghouse Steam Generator Tube Mechanical Plugs.
The subject bulletin requested that the addressees determine whether certain mechanical plugs supplied by Westinghouse are installed in.their steam generators and if so, an action plan be implemented to ensure that these plugs will continue to provide adequate assurance of reactor coolant system (RCS) pressure boundary integrity under normal opertting, transient, and' postulated accident conditions. The actions requested from this bulletin, with the-exception of Action 2e, apply only to Westinghouse mechanical plugs which have been fabricated from thermally treated Inconel 600, heat numbers 3279, 3513, 3962 and 4523.
After reviewing Bulletin 89-01 and the tube plugs supplied to Davis-Besse, Toledo Edison has determined that no tube plugs were procured or installed in the Once Through Steam Generators from heat numbers 3279, 3513, 3962 and 4523.
Paragraph 2e of the Bulletin stated that, addresses should examine Westinghouse mechanical plugs removed from steam generators, regardless of heat number, for primary water stress corrosion cracking (PVSCC) on a sample basis for each heat.- Addressees should maintain a record of these examinations and the results should be provided to Westinghouse to improve the database concerning the susceptibility of plugs to PVSCC.

However, Toledo Edison has procured and installed a Babcock & Wilcox (B&W) plug from heat V592-1 which has been identified by B&V as possibly being susceptible to PUSCC. Babcock & Wilcox has informed the NRC of this abnormality by letter dated September 6, 1988, and also in a meeting held with the staff on April 21, 1989, concerning once Through Steam Generators Tube Plugs. Toledo Edison is actively participating in discussions with B&W and evaluating the information as it is made available by B&V. Toledo Edison intends to review the final B&W disposition and to plan the appropriate recommended action, upon its finalization.
