(NRC INSPECTION REPORT NOS. 50-338/89-05AND50-339/89-05)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
C. H. Bassett ' on March. 20-24, 1989.
The inspection included a review of-
activities authorized for your North Anna facility.
At the conclusion of the
. inspection, the findings were ' discussed with those members of your staff-
identified in the enclosed Inspection Report.
Areas examined during the inspection are identified in the report. .Within
these areas, the. inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The inepection . findings indicate that certain activities appeared to violate
NRC requirements.
The violation, references to pertinent requirements, and
elements to _ be included in your response are described in the enclosed Notice
of Violation.
We believe that the numerous incidents involving failure to
comply with the Technical Specification requirements for entry into high
radiation areas are indicative of-' a serious lack of management control.
Although management attention has been directed toward this matter on several
occasions in the past, the actions to correct the problem have been inadequate.
Efforts to preclude these unauthorized entries appear not to have been
adequately communicated to the workers in the plunt thereby contributing to an
apparent disregard for related requirements by plant and contractor personnel.
Although these incidents have been investigated, it appears that the root cause
has not yet been determined, since similar events continue.
We, therefore,
_
have scheduled a management meeting with you on April 26, 1989, to be held in
the Region II Office.
At this meeting, and in your response to this letter,
you should outline the root cause of this recurring problem, as well as actions
you have taken or plan to take to increase the plant staff's awareness of or
sensitivity to radiological control requirements and to prevent recurrence.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
