MISPOSITIONING OF " REVERSE ACTING" VALVE CONTROLLERS I

A copy of the subject AE0D technical review report is enclosed. This study was initiated in response to an event at Susquehanna Unit 2 which involved controllers of service water regulating valves for room coolers. The investigation of this event of mispositioning of the " REVERSE ACTING" valve controllers found:

If a loss-of-coolant-accident occurred while the valve controllers were mispositioned, and the accident made the controllers inaccessible, then the LPCI and core spray system could become inoperable because of overheated switchgear.

2.

The licensee's corrective actions were adequate and appropriate.

A data search for similar events of mispositioning of " REVERSE ACTING" controllers causing both trains of safety systems to become inoperable during postulated accidents was negative. Therefore, this report finalizes and concludes the investigation of the event, n
