MISPOSITIONING OF " REVERSE ACTING" VALVE CONTROLLERS I

A copy of the subject AE0D technical review report is enclosed. This study was initiated in response to an event at Susquehanna Unit 2 which involved controllers of service water regulating valves for room coolers. The investigation of this event of mispositioning of the " REVERSE ACTING" valve controllers found:

I. If a loss-of-coolant-accident occurred while the valve controllers were

, mispositioned, and the accident made the controllers inaccessible, then the LPCI and core spray system could become inoperable because of overheated switchgear.

2. The licensee's corrective actions were adequate and appropriate.

A data search for similar events of mispositioning of " REVERSE ACTING" controllers causing both trains of safety systems to become inoperable during postulated accidents was negative. Therefore, this report finalizes and concludes the investigation of the event, n
