INADEQUATE MECHANICAL BLOCKING OF VALVES Enclosed is an AEOD Engineering Evaluation report on the inadequate blocking of automatic valves. This misapplication of mechanical blocks was identified as a human performance problem. Accordingly, we are forwarding this report for your infonnation to determine if the maintenance and surveillance plan adequately addresses the human performance problems identified in this report.
