March 31,1987 AEOD/E706 MEMORANDUM FOR: Mr31T5fdtdsM"DfficToYt-Division of Emergency Preparedness and Engineering Response Office of Inspection and Enforcement FROM:

Frederick J. Hebdon, Deputy Director Office for Analysis and Evaluation of Operational Data

SUBJECT:

INADEQUATE MECHANICAL BLOCKING OF VALVES Enclosed is an AEOD Engineering Evaluation report on inadequate blocking of automatic valves. Although this study found that the misapplication of mechanical blocking devices were infrequent, unrepetitive occurrences, a high proportion of the events could have or did result in significant compro-mises in safety. Accordingly, it is suggested that IE consider issuing an information notice to describe several of these events and their underlying causes and actions that could be taken to minimize the possibility of these types of problems.

If you or your staff have any questions concerning the enclosed Engineering Evaluation report, pitese contact Ted Cintula of my staff on extension 24434.
