Gentlemen:

Subject: Inspection No. 50-245/89-13

A reactive, unannounced safety inspection was corrhv+M by P. O'Cbnnell of

this office, on May 17-18, 1989, of the shipment of a contaminated high

pressure pump and trailer from Millstone Unit 1 to Morristown, New Jersey.

After arriving in Morristown, New Jersey the equipment was found to have

removable, external and internal radioactive contamination. The inspection

consisted of selective examination of procedures and representative records,

interviews with personnel, and observations by the inspector. The findings of

the $nspection were discussed during an exit meetirq with Mr. J. Sullivan on

May 18, 1989 and during a <mhc:ryuent telephone Conversation with Mr. J.

Sullivan on May 23, 1989. The details of the inspection are contained in the

attached inspection report.

Based on the results of this inspection, it appears that certain of your

activities were not conducted in full compliance with NRC requirements.

Specifically, on May 11, 1989 a vendor's high pressure pump and tag-a-long

trailer were released from the Millstone Nuclear Generating Station. A survey

to detect radioactive contamination was not performed on the apparatus, which

was contaminated with licensed material, prior to release from the facility.

On May 12, 1989 the apparatus arrived at the vendor's site. Three days later

the vendor, performing an incomirg equipment contamination survey, found loose

surface contamination on the equipment and trailer deck.

Of concern to the NRC is your lack of c3ntrol in this area which resulted in

failure to ship the apparatus in conformance with transportation requirements.

Specifically, you did not identify the type, quantity, or form of licensed

material released from your facility; you did not provide proper instructions

for safe handlirg of the shipment including, shipping papers, labeling, and

placarding; and you released the apparatus with contamination levels well in

excess of regulatory requirements. This resulted in a substantial potential

for personnel contamination and a minor potential for the spread of

contamination offsite.

Based on' the results of this inspection arri the telephone discussion between

Dr. Ronald R. Bellamy of this office and Mr. Stephen Scace on June 9,1989, we

have scheduled an Enfour.w.:ut Conference in the Nuclear Regulatory nn-innion,

Region I office, to discuss these matters with you. That conference has been

scheduled for June 21,1989 at 1:00 p.m.. The conference will provide you and

.your staff an w d. unity to discuss the failures in your management control

system which allowed these violations to occur, and your short term and long

term corrective actions. In addition you should d4= m c the safety

significance of this event. The need for and the nature of appropriate

enforcement actions for the apparent violations will be the subject of

separate couuspordence at a later date.
