NRC Enforcement Action EA 87-213, issued March 17,
- 1988, Violation A,'
' Failure to Follow Procedure,' Example 6.c, cited the licensee for a welder performing a weld which he was not qualified to perform.
The licensee's y
response dated April 14, 1988, stated that corrective cteps, which included qualification review by supervisors prior to assignment of welders, would be achieved by June 15, 1988.
Contrary to the above, the corrective actions were not effective in precluding repetition in that on March 24,
- 1989, the licensee identified that a vanqualified welder had performed welding on a portion of safety-related picing, in November 1988, specifically the essential service wster j
(ESW) return to the ultimate heat sink.
Reason For Violations The Mechanical Maintenance Supervisor designee failed to verify the welder's qualifications in accordance with procedure ADM 08-302,
" Welder Filler Material Control".
Disciplinary actions were taken for the personnel involved.
Corrective Steps Which Have Been Taken And Results Achieved:
Upon identification of this condition, a letter (MA 89-0098) was issued on March 24, 1989, which required an independent verification of welder qualification be performed by Welding Engineering personnel for all safety related and special scope welding.
Welding Engineering personnel performed a review of safety related and special scope welding that had been performed by the nonqualified welder.
This review identified two additional special scope welds that had been installed by this welder.
The replacement of these welds is discussed below.
Corrective Stens Which Vill Be Taken To Avoid Further Violation 3a Procedure ADM 08-302 and the Authorized Signature List (General Maintenance and Modifications Information Bulletin No.
- 11) was revised to require Welding Engineering personnel to verify welder qualification by authorizing the Field Welding Material Requisition after job assignment by the l
supervisor and prior to issuan t of the weld filler material.

Although.the welds have been determined to be. '
sound welds' using nondestructive examinations as required by the applicable code, strict code compliance for welder qualification can not be demonstrated.. Therefore, the-welds will be. removed and replaced as follows:
(1) Base metal repair on ESW "A" Train safety related' warming line has-been documented on Corrective Work Request 01488-89 and will 'be removed and replaced by Septembe/ 29, 1989.
(2) 'The Shielded Metal Arc Welding (SMAW) portion of.two (2) four inch butt welds in line BM140-EBD-4" (the special scope portion of the Steam
. Generator
. Blow Down System) has been documented 'on Corrective Work Request 01506-89 and replacement of these special scope welds requires'an outage.
The special scope welds will be t-removed and replaced. prior to startup from-the fourth refueling outage.
Date-When Full Compliance Will Be Achieved:
All programmatic changes have been completed. Replacement of the ESW safety related warming line weld will be completed by September. 29, 1989 and replacement of the Steam Generator Blowdown special scope welds.will be completed prior to startup.from the fourth refueling outage.
