This refers to our letter dated March 17, 1989 concerning your response to allegations involving the fitness for duty of one of your employees. Attached to our letter was a redacted version of your report dated September 23, 1989.
Our further review has shown that Page 6 of the report attached to your letter contains additional information that should be protected. Attached to-this letter is a copy of Page 6 that has had the protected information removed.

Response
performance evaluations indicated a decline in performance from T986 to 1988. Hovever, these evaluations all indicated at least acceptable levels of performance.
As de_termined through the interview process, concerns existed among subordinates.
Each person interviewed was questioned about any possible impact involving plant safety. As this would relate to f
each_ferson felt it was not possible that errors would go undetected Those interviewed were Mespons1 Die for the technical sign-offs.
These same persons, however, stated!
did adversely impset morale during the time in question.

Response
During the, interviews, it van determined that various people observed mood changes, lack of ability to concentrate at times and alcohol on his breath on more than one occasion.
One _
reported observing
/ complete loss of attention which f attributed to a pre-existing injury from a traffic injury.
This injury could not besubstantiatedbymedicalinformationandf declined to comment on this injury. I personal psychiatrist has indicated that this inability to concentrate is a symptom of L diagnosed condition L. _

Itanes of partles and certain other fdentlfying details have been renoved in order to prevent a clearly unwarranted invasion of the personal privacy of the Individuals Involved.
