VIRGINIA ELECTRIC AND POWER COMPANY NORTH ANNA POWER STATION UNIT 1 STEAM GENERATOR TUBE RUPTURE EVENT REPORT REVISION 1 Virginia Electric and Power Company letter dated July 29, 1987 provided our initial report on the North Anna Unit 1 Steam Generator Tube Rupture Event. We have completed our evaluation of this event and revision 1 to the subject report is being provided for your review. The report includes the results of our investigations of the tube failure mechanism and identifies corrective actions being taken to assure the long term integrity and operab111ty of the steam generators. The report also includes the findings from our evaluation of the response to plant equipment, personnel and procedures during the event.
Corrective actions are identified addressing each of the findings in these areas.
In addition. Westinghouse Ele:tric Corporation
- report, STD-7.2.4-7126, entitled, " North Anna 1 Steam Generator Tube Rupture and Remedial Actions Technical Evaluation," is being provided by a
separate letter from Westinghouse.
This report provides details of the investigation and analysis of the tube f ailure mechanism and steam generator corrective actions and repairs.
NRC letter dated July 22, 1987 confirmed our agreement that Virginia Electric and Power Company would obtain concurrence from the NRC prior to Unit I startup (Mode 2).
We believe that the information which accompanies this letter provides a sufficient basis for concluding that Unit I can be operated safely.
We request your expeditious review and concurrence with returning Unit 1 to service. As it is recognized that the NRC review of this matter may extend beyond late September when Unit I will be ready for startup, we will be requesting your approval for startup and operation at reduced power (50% of rated power). This request will be submitted by September 21, 1987.
