G SPECIAL REPORT DIESEL GENERATOR 1B VALID FAILURE On January 19, 1986 a valid failure occurred on Diesel Generator (D/G) 1B.

This valid failure was the fifth failure within the last 75 valid tests for the Catawba Unit 1 Diesel Generators.

Please note that as of January 19,1986,thI 1A and IB Diesel Generators had only completed 75 valid tests (this includes t$e...

five valid failures).

-The failure occurred while the IB D/G was running in the fully loaded condition.33 During the test, the fuel supply make-up solenoid valve-failed to open and ed.;

provide. fuel for the IB D/G day tank.

Thirty-three minutes into the test, thN 1BL D/G tripped due to a lack of fuel. To correct the malfunction, the IB D/G da3 tank manual bypass supply valve was opened to refill the day tank. In additiMn, an auxiliary fuel oil pump was used to refill the IB D/G fuel supply piping. The IB_D/G was restarted and verified operable by performing the IB D/G operability periodic test. The 1B D/G was declared inoperable on January 19, 1986 at 2245 and was subsequently declared operable on January 20, 1986 at 1600. Therefore, the 1B D/G was out of service for 17 hours1.967593e-4 days <br />0.00472 hours <br />2.810847e-5 weeks <br />6.4685e-6 months <br /> and 15 minutes. At the time, the surveillance testing interval was 3 days which was in accordance with Section C.2.d.4 of Regulatory Guide 1.108 Revision 1 (Periodic Testing of Diesel Generator Units Used as Onsite Electric Power Systems at Nuclear Power Plants).
