This letter addresses the inspcction of your facility at Montebello, California, conducted by J. C. Harper of this office on May 24-29, 1987, and the discussions of his fir. dings with James Lastra and other members of your staff at the conclu-sion of the inspection.
The purpose of the inspection was to determine the adequacy of the Precision Heat Treating (PhT) (utlity Control / Assurance Program, and heat treating practices pertaining to open fire tempered and quenched products.
As a result of uncontrolled heat treatment, the now defunct Cardinal Industrial Products Corporation (CIPC) supplied Commonwealth Ecison (CECO) with fastencrs possessing mechanical properties lowcr than allowed by material specification.
PHT was the subcontracted heat treater for CIPC.
In general it appears that the PHT heat treating practices pertaining to open fire tempered quenched products were properly carried cut. The PHT heat treat records for the CECO fasteners indicated that the fasteners were correctly heat treated. However, from interviews with PHT personnel and documentation reviews, the NRC inspector was informed that or occassions CIPC would submit for heat treating, fasteners that had been previousb heat treated togethe.r with fasteners that had not been heat treated. The CEC 0 fasteners may have received an undesirable heat treatrent as a result of being reheat treated by PHT.
In summary, not enough information is available to conclusively identify the origin cf the problem with the CECO fasteners. However, complete QA documenta-l tion regarding heat treating and quenching times and temperatures, exists et PHT indicating that heat treatment of these fasteners was accomplished acccrding to specifications.
Areas examined during the NRC inspection end our findings are discusst.d in the enclosed report. This inspection ccr.sisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.

During this inspection it was found that the implementation tf your QA program failed to meet certain NRC requirements.
It was determined that PHT did not have a Part 21 procedure and failed to properly record pertinent information on fJrnace charts. The specific findirp and references to ti.e pertinent te @ ements are identified in thA. enclosures to this letter.
The encimed htice of Violation is/sent to you pursuant to tie provisions of l-Section 106 of the Energy Reorganization Act of 1974. You are required to submit 7.o this office within 30 days from the date of this letter a written statelnent coniaining~:
