MONTHLY OPERATING ACTIVITIES - NOVEMBER 1987 HIGHLIGHTS l

Unit 2 entered the reporting period recovering from condensate system high oxygen.

Reactor power had been lowered due to the high oxygen, and was restored to 80% at 0330 on 11-1-87 following identification and repair of the l

oxygen in leakage.

Condenser water boxes were cleaned due to rough lake conditions without reducing reactor power.

The reporting pdriod ended with Unit at 80% of rated thermal power.

Gross electrical generation for the month of November was 642,370 MWH.

SUMMARY

11-19-87 1748 Declared power range detector (N-43) inoperable due to a bad coaxial cable on detector B.

All N-43 bistables were tripped.

11-20-87 0548 Entered technical specification 3.0.3 due to inoperable powerrangedetector(N-43). Reactor power was greater than seventy five percent, (actually eighty percent),

while the high flux setpoint was not reduced to eight five percent. Quadrant power tilt ratio was not verified within the last 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> due to removing N-43 from service.

11-20-87 0605 Exited technical specification 3.0.3 power range detector, N-43, was declared operable.

Puplis TELEPHONE (616) 465-5901 PAGE 1 of 1 MAJOR SAFETY-RELATED MAINTENANCE NOVEMBER, 1987 M-1, M-2, All four steam generators had some damaged manway bolt M-3, M-4 holes. A total of 34 bolt holes were repaired with helicoils.

M-5, M-6 2-lM0-212 (east containment spray pump discharge to containment spray additive eductor shutoff valve) had a Body to bonnet leak and would not seat properly in the closed position. The stem was found to be bent. The stem, packing and gasket were replaced. The valve tested l

satisfactorily.

M-7 2-IRV-260 (safety injection test line shutoff valve) would not I

stroke. The actuator diaphragm was replaced and the seat was lapped. The valve tested satisfactorily.
