Routine Inspection 50-245/87-12 (6/23/87 to 8/10/87)

This transmits the report of the above subject inspection of Millstone Unit 1 by

Mr. G. Grant of this office. The inspection findings have been discussed with

Mr. S. Scace of your staff. No violations were identified,

As Detail 4.4 of the enclosed report notes, we found your initial acti?ns in

response to IE Bulletin 84-03 to be conservative, well-engineered, and timely.

It appears, however, that you have not identified to the NRC the additional

follow-up action to which you committed.

Incident to that, it also appears

that you have not provided formal instructions to the operators on the actions

to be taken if fuel is being moved and the refueling cavity water levn1

decreases significantly. Although we recognize that failure of the ' metal

bellows seal at Millstone 1 is far less likely than failure of a pneumatic

seal, we are concerned about the apparent lack of follow-up on commitments you

.

made in 1984 on this matter.

Therefore, please reply in writing to this letter

l

within 30 days of its receipt, stating the reasons for not identifying 'your

'

corrective actions, your present plans to complete them, and your plans'to

assure that such commitments are carried out in the future.

We have received your June 25, 1987 reply to two Violations for untimely sub-

mittals of amendment requests. These submittals addressed primary containment

isolation valves (TS Table 3.7.1) and hydraulic and mechanical snubber lists (TS

Tables 3.6.1.a and 3.6.1.b).

This inspection report reviews these issues in Sec-

tions 4 and 7.

In addition, we note your assessment of the generic timeliness

problem and your proposed corrective actions. We found your actions responsive

to our concerns, and we will further evaluate the long term trend of this issue

during routine inspections.

We also received your July 17, 1987 letter on corrective actions on reactor trips

l

attributed to the reactor pressure control system.

The improvements in operating

'

procedure guidance, operator requalification training, MPR/EPR practice transfers,

MPR/EPR modification investigation, MPR overhaul, and human factors redesign of

various MPR/EPR control board indications are responsive to our concerns.
