NOTICE OF VIOLATION Illinois Power Company License No. NPF-62 As a result of the inspection conducted on June 30 through August 4,1987, and in accordance with the " General Policy and Procedure for NRC Enforcement Actions," 10CFR Part 2, Appendix C (1985), the following violations were identified:

1. 10 CFR 50, Appendix B, Criterion XVI, as implemented by Illinois Power  :

Company Operational Quality Assurance Manual (00AM), Chapter 16,  ;

" Corrective Action," requires that conditions adverse to plant safety l and/or quality are promptly identified and corrected; and that significant conditions are evaluated, documented, corrected, reported, and independently reviewed.

Contrary to the above:

Between March and July, 1987, numerous discrepancies, thought to be limited to only " housekeeping" items, were identified by the licensee.

The licensee failed to promptly correct conditions and/or failed to evaluate and document the discrepancies that had been identified during this period.

This is a Se,4 :ty level IV violation (Supplement I) (461/87031-04(DRP)).

2. Clinton Power Stat n Technical Specification 6.8.1 requires, in part, that written proceaures shall be established, implemented, and maintained covering the activities . . . recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978. Operating Procedure CPS No. 3304.01V001, Revision 5, " Control Rod Hydraulic And Control Valve Lineup," required Valves IC11-F155A, B, C, and D, and IC11-158A, B, C, and D to be locked open, and Valve IC11-F361A was required to be locked closed.

Contrary to the above:

On July 21, 1987, Valves IC11-F155A, B, C, and D; IC11-F158A, B, C, and D; and IC11-F361A were found to be missing their locking devices.

This is a Severity Level IV violation (Supplement I) (461/87031-05(DRP)).

3. Technical Specification 4.6.4.1 requires that the containment isolation valves shown in Table 3.6.4-1 be demonstrated operable prior to returning the valve to service after maintenance . . . by cycling the valve and verifying its specified isolation time.

Contrary to the above:

On June 4,1987, Containment Isolation Valve IVR001A was identified as not having been demonstrated operable (the required stroke time test had not been performed) prior to entry into the applicable operating condition. (461/87031-07a(DRP)).

On July 10, 1987, Containment Isolation Valve IVR001B was declared operable, while in the applicable operating mode, without demonstrating its operability by performing the required stroke time l test. (461/87031-07b(DRP)).

This is a Severity Level IV violation (Supplement I).

4. Clinton Power Station Technical' Specification 3.7.2 requires, in part, that in Operational Condition 1,'2 , or 3 with one Control Room Ventilation System inoperable, restore the inoperable system to operable status within seven days or be in at least HOT SHUTDOWN within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> and in COLD SHUTDOWN within the following 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

Contrary to the above:

Control Room Ventilation System Train-A was made inoperable on June 23, 1987, and remained inoperable until July 15, 1987. From June 23 to July 15, the plant was in Operational Condition 1, 2, or 3. The Action Statement requirements were not satisfied within the time allotted by the Action Statement.

This is a Severity Level IV violation (Supplement I) (461/87031-12(DRP)).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good'cause shown.
