Purpose:

This information notice is provided to alert recipients to a potentially significant safety problem pertaining to the use of anaerobic adh*7sjvesand sealants.

It is expected that recipients will review the inforoaSig for applicability to their facilities and consider actions, if approprhte, to preclude a similar problem. However, suggestions contained fn this notice do not constitute NRC requirements; therefore, no specific acti, orcwritten response is required.

1 Description of Circumstances:

j On July 1,1987, Carolina Power and Light Company (CP&L) reported that follow-ing a reactor trip / turbine trip at Brunswick Steam Electric Plant, Unit 1, t

(Brunswick) one safety relief valve (SRV) failed to open then manually actuated for pressure control.

The SRV is part of the automatic deprtssurization system J

(ADS) at that plant. Subsequent testing on July 3rd of othe( ADS valves resulted in a second valve failing to open on manual actuat ko. During post 1'

t, failure examination it was determined, that during maintenance, Ldtite was 1

used to help seat a locknut in the solenoid assembly and this,t

. material migrated

,U to the solenoid clearance around the plunger prior to setting In this in-I stance, the material was Loctite RC 620. The valves at' Brunswick were recently

( [

rebuilt by Target Rock Corporation at Wyle Laboratory. At Brunwick, the

't failures were discovered shortly after the valves were installed and aiuy;-

containment was inerted.

5 Discussion:

This event is similar to that described in Information Notice 84-fil "Informa-tion Concerning the Use of Loctite 242 and Other Anaerobic Adhesf.ve/ Sealants."

In that event, Loctite 242 Threadlocking Adhesive / Sealant was u5 d in the assembly of scram pilot solenoid valves. An investigation into the failure of several scram solenoid valves revealed the cause to be the failure of natnte-nance technicians to wipe excess Loctite from the assembly which 6tnolted in j

the bonding of the solenoid core riunger to the base assembly.'

l RC 620 was used. This sub:tance is anaerotic (cures in the absence of air).

i Thus, the plunger did not become seized unt 4 hfter the valves with excess Loctite were plac?d in the inerted atmosphes! which exists in the reactor 1

containment when the reactor is brought td)ower conditions.
