White Manager of Nuclear Power 6N 38A Lookout Place 1001 Market Street ' Chattanooga, TN 37402-2801 Gentlemen: SUBJECT: SPECIAL TEAM INSPECTION OF EMPLOYEE CONCERN ELEMENT REPORTS -- SEQUOYAH INSPECTION REPORT NOS. 50-327, 328/87-23 An NRC team conducted a special announced inspection of activities authorized by NRC Operating License Nos. DPR-77 and DPR-79 for your Sequoyah facility on April 6 through 17, 1987. At the concitsion of the inspection, the findings were discussed with those members of your staff identified in the enclosed inspection report. Areas e_xamined during the inspection are identified in the report. The inspection consisted of selective examination of Employee Concern Task Group (ECTG) Quality Assurance (QA) Element Reports for Sequoyah (SQN), procedures and representative records, confirmatory field inspections, and interviews with personnel. Within the scope of the inspection, one violation of NRC requirements for complyirg with established quality assurance instructions was identified in an engineering verification of instrument mountings. On the basis of the results of this inspection, Safety Evaluation Reports (SERs) will be issued without further review or action for the following Element i l Reports: 80102-SQN, 80303-SQN, 80401-SQN, and 80402-SQN. SERs will be issued for the following Element Reports after our review of revised reports for incorporation of changes required by this inspection and documented in the attached inspection report: 80000-SQN, 80101-SQN, 80201-SQN, and 80203-SQN. The following Element Reports require additional information and NRC review before the SERs can be issued; 80202-SQN, 80501-SQN, 80503-SQN, and 80504-SQN. Before this inspection, SERs were prepared for Element Reports 80104-SQN, 80205-SQN, and 80404-SQN. Several observations regarding effectiveness of programs, practices requiring management attention, and management attitudes were made during our reviews and inspections. Generally, the process established and being implemented by TVA for preparation of Element Reports was considered acceptable. No apparent QA program corrective actions in resor we to the large number of installation deficiencies, identified during an engineering calculation, indicate a less than effective QA involvement in this area. Management attention is required for the practice of using an electrical calculation instruction for l the design / field verification of instrument mountings and for the implications l of NRC identified deficiencies in this area to other similar work. Our inter- views of QA auditors indicate that Sequoyah management attitudes toward QA audits and the value of audit findings have improved over the last few months. .In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, l Title 10, Code of Federal Regulations, a copy of this letter and its enclosures will be placed in the NRC Public Document Room. f ' You are requested to respond to the enclosed Notice of Violation and to address the NRC observations noted above. The responses directed by this letter and . its enclosures are not subject to the clearance procedures of the Office of ' Management and Budget as required by the Paperwork Reduction Act of 1980 (PL 96-511).
