SUBJECT: NRC INSPECTION REPORT 50-458/98-15
Dear Mr. Edington:
From June 28 through August 8,1998, an NRC inspection was conducted at your River Bend
Station reactor facility. The enclosed report presents the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at River Bend
Station was generally characterized by safety-conscious operations, sound engineering and
maintenance practices, and effective radiological controls. Your response to a feedwater flow
venturi instrument line crack reflected excellent management involvement which, in turn,
resulted in well controlled and coordinated maintenance.
We also noted that you have identified two negative performance trends. First, Operations
personnel continued to make errors that have impacted plant performance. Your initial
corrective measures for this trend were not timely in that detailed crew performance briefings
were conducted approximately one month after the trend was identified. Second, you've
identified severalinstances of surveillance that were not performed when required. For both of
these performance trends we consider your planned and completed corrective measures
adequate and reasonable. Nonetheless, we encourage you to maintain effective managoment
oversight of these areas to prevent further recurrence.
'In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosure, and your response will be placed in the NRC Public Document Room (PDR).
