SUBJECT: ' NOTICE OF VIOLATION

'(NRC INSPECTION REPORT NOS. 50-327/87-66 AND 50-328/87-66)

This refers' to a special Nuclear Regulatory ' Commission (NRC) system alignment

-

inspection conducted - by J. B. .Brady on October 19-30, 1987. The inspection

~

included a review of' activities' authorized for your Sequoyah facility in order.

to' verify that. the. system alignment and -configuration control process are

adequate to support mode change.

At. the ' conclusion of the ' inspection, the

findings were discussed with those members of your staff identified in the-

enclosed inspection report.

The inspection findings indicate that certain activities appeared to. violate

.NRC requirements. The violations, references to pertinent requirements,.and

elements to be included in your response are described in the enclosed Notice

of Violation.

Violation A involves six examples where administrative

, requirements for the system alignment and configuration control process were

not properly established, implemented or maintained as required by Technical

' Specification 6.8.-l.

Violation B involves three examples where your System

Operating Instruction (SOI) for the Emergency Core Cooling System was inade-

quate. This is contrary to Technical Specification 6.8.1 for establishing,

implementing, and maintaining written procedures.

We concluded that the system alignment and configuration control process as

implemented were inadequate to support mode change. Additionally, we concluded

that additional NRC inspection would be required following resolution of

deficiencies and prior to Mode 4 entry.

We;are concerned that you would initiate and carry out programs as important as

system alignment and configuration control without having previously addressed

the problem areas identified in the violations on your own initiative.

The

ability to adequately conceive, develop and implement programs such as these is

an'important element in assuring readiness.fo'r restart of Sequoyah Unit 2.

You

should ensure that this concern is addr'essed in your response to the Notice of

' Violation. Also please be advised that we will closely monitor your performance

in completing the system alignments, preparing for heatup, and entering into

and operating in Mode 4 in order to access your resolution of this concern

prior to unit startup.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosures

will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosures are not subject to'the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
