Dear Mr. MacDonald:
During a recent NRC field audit, the auditor discovered that our company was not registered with your branch of the NRC as a shipper using the Amersham - Tech / Ops devices we presently use.
It was suggested that we contact the Amersham Corporation for information as to the necessary action we would have to take.
One of the provisions in the information Amersham provided indicates a requirement for NRC approval of the Companys' Quality Assurance Program. We believe this approval was obtained May 23, 1984, under your approval #0518 shortly after the Licensing Section issued our radiography license; however, photo copies of the approval, related correspondence end our Quality Assurance Program are attached. Page 5 of 5 of the Quality Assurance Program shows a shipping contained check list. I use this document in checking out source shipping records and sign off this check list during each quarterly audit.
Two letters requesting that the Company be registered as a user of the Tech / Ops 650 and 660 devices are attached also.
