ENCLOSURE Violation 50-327/84-24-01, Example f i
" Radiation Work Permit (RHP) 02-1-00102 issued January 1, 1984, for seal table-area inspection and maintenance required workers to verify the presence of a clearance on the incore instrument probes prior to entering the containment lower compartments and annulus.
This requirement was not implemented on April 18-19, 1984, by workers entering the seal table area in that the clearance was not in effect on the problems during work activities.
l Corrective Actions:
The RHP procedure and RHP cover sheet have been revised to require the RWP timesheet to be removed when incore probes are in use. AI-8 also contains requirements for having the RHP timesheet approved and in place for maintenance activities on the moveable detector system.
NRC Comment:
(While this corrective action appears adequate and relative to the issue, no evidence can be found of the implementation of the corrective action in a review of the RHP procedure RCI-14.
If later revisions have removed this stipulation, failure to maintain commitments may be indicated)."
Corrective Action Taken SON Administrative Instruction (AI)-8, " Access to Containment," was revised to read:
Priortoentryintolowercontainmentortheannblustheincore flux detectors shall be verified to be in the storage position or inserted to within ten (10) feet of the core.
The SE shall initiate a hold order clearance on the incore flux drive motors control power.
This hold order shall remain in effect until the SE is assured all personnel have been cleared from containment and the personnel access is locked.
Prior to issuing a radiation work permit (RHP) for lower containment or annulus, RC shall verify i
i incore detector system is tagged with a hold order. The SE will issue incore detector system hold order clearance to RC Shift Supervisor by title.
The incore detector hold order clearance will remain issued to RC Shift Supervisor by title at all times except when running core maps or performing incore detector system maintenance that requires the detector system to be operated while persons are in the incore instrument room.
i Maintenance work that requires the incore detector system to be operated while persons are inside the incore instrument room shall be coordinated by Operations, RC, and the applicable maintenance i

. The SE will contact RC and Public Safety to verify no one inside containment or annulus and Public Safety will verify access is locked prior to releasing clearance on incore detector system except as outlined above.
In addition, Radiological Control Instruction (RCI)-14, " Radiation Work Permit (RWP) Program," section 8.4.1.3 states, " Entry into containment shall be performed in accordance with AI-8."
The RWP cover sheet was also revised to state that entry into containment will be performed in accordance with AI-8.
At the time of the initial response, RWP cover sheets were not considered to be quality assurance records; and RCI-14 simply stated to use an RWP cover sheet similar to attachment 1.
The changes were made to the cover sheets used in the field, but the attachment in the procedure was not revised.
A copy of the revised RWP cover sheet was provided to the NRC Resident Inspector for review.
The revision to AI-8, along with revising the RWP cover sheet, ensured SQN had completed the corrective actions as specified in our initial response to this violation. After reviewing the appropriate documentation, the NRC Resident Inspector agreed with the verification of this item.
