0 REQUEST FOR ADDITIONAL INFORMATION MILLSTONE NUCLEAR POWER STATION. UNIT 3 l

DOCKET NO. 50-423 l

1.

Proposed revision to Final Safety Analysis Report Section 6.4.3, states in part that, in the i

event of a loss-of-coolant accident (LOCA), operators are credited with securing selected l

non-nuclear safety grade (NNS) fans within 20 minutes following at least 1 hour1.157407e-5 days
2.777778e-4 hours
1.653439e-6 weeks
3.805e-7 months
of operation in the pressurization mode. After 1 hour1.157407e-5 days
2.777778e-4 hours
1.653439e-6 weeks
3.805e-7 months
, control room ventilation can be realigned from pressurization to recirculation mode. Describe what evaluations were performed to verify that the operating crews were capable of successfully performing the required actions to stop the selected fans within the 20-minute time frame specified.

2.

Describe the indications and controls (including location) that are relied upon by the operators (1) to determine that the fans must be isolated, and (2) to accomplish the required actions. For any actions taken outside of the control room, describe the j

presumed environmental conditions to which an operator would be subjected (heat, humidity, lighting, radiological, etc.), and describe how potential barriers to successful I

l operator performance were considered and dispositioned.

3.

Describe any procedural modifications that were implemented to direct the operators to l

secure the NNS fans, and describe the training that was provided to the operators.

4.

What is the conditional probability of the initiating events (i.e., LOCA without loss of offsite power) that would require the operator to perform the NNS fan isolations. Dercribe what consequences would result if the operators were incapable of isolating the NNS fans within 1 hour1.157407e-5 days
2.777778e-4 hours
1.653439e-6 weeks
3.805e-7 months
and 20 minutes.

5.

Describe what other breakers or controls important to safety are in close proximity (i.e.,

adjacent to or within the normal reach of the operator) to the NNS fan controls. If these other breakers or controls were inadvertently manipulated, describe what, if any, effects it could have on the successful mitigation of the event.
