SUMMARY

OF MEETING TO DISCUSS CAUSES AND MODIFICATIONS RELATIVE TO PIPE FAILURES IN THE AUXILIARY FEEDWATER SYSTEM AT SOUTH TEXAS PROJECT, UNIT 1, ON DECEMBER 2, 1987 The meeting attendees are shown in Enclosure 1. The. licensee provided handouts which are included herewith as Enclosure 2.

Discussion:

During startup of the South Texas Project, Unit 1, piping (vent, drain or instrument lines) and pipe supports associated with the auxiliary feedwater system experienced failure on at least three occasions. The licensee undertook extensive testing to determine the causes for the vibratory forces which led to the failures. The presentation provided details on the results of the tests and post-failure examinations, and also included proposed preventative actions.

Subsequent to the presentation, the licensee was informed that the staff could not conclude that sufficient testing had been done or that the root causes had been adequately addressed. The licensee indicated that further testing would be performed to support a root cause determination.

IN THE PRESENCE OF AN ESF SIGNAL (SAFETY INJECTION, 2/4 LO-LO LEVEL IN ANY STE AM GENERATOR OR AMSAC INITI ATE CONTROL ) WILL. THRU GDPS. OPEN THE VALVE TO PROVIDE APPROXIMATELY 600-625 GPM FLOW.
