' De:r Mr. Lamastra, This letter serves as formal notification of the event discussed via telephone this day.

As a coudesy notification, it is neither required by procedure or regulation.

At approximately 1205, I was notified of a vehicular accident on the premises. The information provided indicated the incident involved a single, unoccupied vehicle. This vehicle, a tractor-trailer rig, had been positioned in front of a double gate located in the northwest corner of the fenced site perimeter. The trailer

belongs to Framatome Technologies, and houses a steam generator chemical decontamination system. The driver reportedly set the trailer brake, but not the brake of the tractor, then departed to the SERF 3 facility.

The single reservoir brake system of the trailer bled down, and the rig began to roll. The mechanism for securing the gate was overcome. The rig continued on the property for approximately 150 feet at which point it had jackknifed with the trailer positioned approximately 180 degrees from its original orientation.

j The front end of the trailer came to rest against a tree. The left front wheel of the tractor came to rest rgrinst an cadh berm created during this event.

As the Emergency Officer,I placed the Haz-Mat team and Fire Brigade on standby. Fire extinguishers were positioned in the vicinity of the rig. An evaluation of the rig found a fuel leak rate of approximately one drop per 1-2 mimites via a tank cap vent hole. No other leakage was observed. The tr ctor incurred light to moderate scratches and some minor denting. It was later determined that a power steering hose had also been ruptured. The trailer and its contents were unharmed. This was accomplished by 1900 hours0.022 days <br />0.528 hours <br />0.00314 weeks <br />7.2295e-4 months <br />. Off-t site supped was required to extricate the rig. The tractor was later repaired. Both the tractor and the trailer 0

- were inspected by a certified mechanic and declared road worthy.
