Enclosed is Addenda A to the LaSalle Unit 1 Inservice Inspection (ISI) program. This addenda corrects inadequacies found in the existing i program (reference (a)). The necessity for this change and detailed i discussion of each aspect is included in the attachment. l The revisions provided in the addenda improve the quality of the LaSalle Unit 1 ISI program. The addenda will be implemented prior to the Unit 1, second refueling outage which is currently scheduled to start in March 1988.

Commonwealth Edison is notifying the State of Illinois of our request for this change to the ISI program by transmitting'a copy of this letter and attachment to the designated state official.

ATTACHMENT Description of Addendum A of the LaSalle Unit 1 Inservice Inspection Program i

The addenda is necessary as the existing Inservice Inspection l program (Rev. 1, dated June 3, 1982), is lacking in the following areas: j i

the program does not include all high stress welds, as required by ASME Section XI. A meticulous review of the piping stress pattern, by architect engineers Sargent & Lundy, has shown the original high stress weld group to be inaccurate;

-- one ASME Class 1 dissimilar metal weld was inadvertently placed in Examination Category B-J. This addenda transfers the weld to the I Category B-F Program in accordance with ASME Section XI; the program does not concentrate inspections on those areas where a relatively high probability of pressure boundary degradation exists. For example, no effort was made to bias the inspection sample towards the examination of stainless steel or inconel welds which have shown a susceptibility to stress corrosion cracking; the program lists many welds for inspection which are in a very high radiation area or which are partially or fully inaccessible for inspection. ASME Section XI permits some flexibility when determining the weld inspection group to allow for ease of inspectability and to help maintain radiation dosage as low as possible; the program contained inappropriate welds in the Augmented Inspection Type 1A Category. Systems not considered to be in high energy line break exclusion regions were listed as such in the Type 1A Category.

This revision affects the weld inspection scope for the ASME Section XI Examination Category B-F Program, the Category B-J Program, and the Category C-F Program. These are, respectively, ' Class 1 Pressure Retaining Dissimilar Metal Welds', ' Class 1 Pressure Retaining Welds in Piping', and ' class 2 Pressure Reta*.ning Welds in Piping'. The revision also affects the weld population of Augmented Inspection Type 1A, 'High Energy Welds'.

. Page 2 of 5 LaSalle Unit 1 is required to follow the 1980 Edition including the Winter 1980 Addenda of ASME Section XI. This Code contains specific requirements for program weld selection in the footnotes of Tables IVB-2500-1 for Class 1 components, and IWC-2500-1 for Class 2 components.

These footnotes are transcribed below.

The requirements for Examination Category B-F are as follows; (1) Examinations are required on each safe end weld in each loop and connecting branches of the reactor coolant system.

(2) For the reactor vessel nozzle safe ends, the examinations may be performed coincident with the vessel nozzle examinations required by Examination Category B-D.

(3) Includes dissimilar metal welds between combinations of:

(a) carbon or low alloy steels to high alloy steels; (b) carbon or low alloy steels to low nickel alloys; (c) high alloy steels to high nickel alloys.

One weld which meets the inclusion criteria of (3)(a) above was discovered, and this addenda adds that weld to the 10-year inspection program under Examination Category B-F. This weld was included as a Category B-J weld in the ISI Program, Rev. 1. A summary listing of those welds which require inspection due to the stipulations of Examination Category B-F is enclosed as a part of the addenda. This list should be incorporated into the LaSalle Unit 1 Inservice Inspection Program under Tab

  1. 11.

Examination Category B-J requires that; (1) Examinations shall include the following:

(a) All terminal ends in each pipe or branch run connected to vessels; (b) All terminal ends and joints in each pipe or branch run connected to other components where the stress levels exceed the following limits under loads associated with specific seismic events and operational conditions:

(1) primary plus secondary stress intensity range of 2.4Sm for ferritic steel and austenitic steel; l l

(2) cumulative usage factor 'U' of 0.4.

(c) All dissimilar metal welds between combinations of:

(1) carbon or low alloy steels to high alloy steels; 3944K i

Pege 3 of 5 (2) carbon or low alloy steel to high nickel alloys; (3) high alloy steels to high nickel alloys.

(d) Additional pipe welds so that the total number of circumferential butt welds (or branch connection or socket welds) selected for examination equals 25% of the circumferential butt welds (or branch connection or socket welds) in the reactor coolant piping system. This total does not include welds excluded by IWB-1220. These additional welds may be located in one loop (one loop is currently defined for both PWR and BWR plants in the 1980 Edition).

(2) The initially selected welds shall be re-examined during each inspection interval.

(3) Includes essentially 100% of weld length.

(4) The examination includes at least a pipe diameter, but not more than 12 in., of each longitudinal weld intersecting the circumferential welds required to be examined by Examination Categories B-F and B-J.

(5) For welds in carbon or low alloy steels, only those welds showing reportable preservice transverse indications need be inspected for transverse reflectors.

Inspections addressed by Note (1)(a) and Note (1)(c) above were already required by Examination Category B-P, and are included under the Category B-F program List.

Note (1)(b) above requires the inspection of all Class 1 high stress welds. All high stress welds are included in the attached Category B-J inspection program revision and are indicated as such in the ' Notes' column.

Note (1)(d) requires the total number of Class 1 circumferential butt welds inspected to be equal to 25% of the total number of non-exempt class 1 circumferential butt welds. These additional welds have been added to the Category B-J Inspection Program to bring the mir.imum number of welds inspected to be at least equal to the required percentage. Because these additional welds are required as a certain size group, not as a specific weld sample, LaSalle reserves the right to substitute welds within that group when inspection difficulties are encountered, as long as all ASME Section XI requirements are satisfied. Welds will only be substituted during the first inspection interval so that Note (2) above is not violated. All weld substitutions will be reported in the Inservice Inspection Summary Report subsequent to the inspection where the substitution occurred.

Note (3) above is adhered to with the exception that some welds are addressed by a previously approved relief request which describes an alternate examination. It this is the case, the relief request will be referenced on the Category B-J program list. The Inservice Inspection Program, Rev. 1, contains all relief requests in their entirety.

a Page 4 of 5 A summary list of all Examination Category B-J welds requiring inspection during each 10-year interval is enclosed. This list should be inserted under Tab #15 of the LaSalle Inservice Inspection Program.

The requirements for Examination Category C-F are as follows:

(1) The welds selected for examination shall include:

(a) all welds at locations where the stresses under the loadings result from normal and upset plant conditions as calculated by the sum of equations (9) and (10) in NC-3652 exceed 0.8(1.2Sh+Sa);

(b) all welds at terminal ends (see (e) below) of piping or branch runs; (c) all dissimilar metal welds; (d) additional welds, at structural discontinuities (see (f) below), such that the total number of welds selected for examination includes the following percentages of circumferential piping welds; ,

For boiling water reactors:

l (1) none of the welds exempted by IWC-1220, 1

(2) none of the welds in residual heat removal and emergency core cooling systems (see (g) below), ,

(3) 50% of the main steam system welds, (4') 25% of the welds in all other systems.

(e) terminal ends are the extremities of piping runs that connect to structures, components (such as vessels, pumps, valves), or pipe anchors, each of which act as rigid restraints or provide at least two degrees of l restraint to piping thermal expansion; l (f) structural discontinuities include pipe weld joints to

' vessel nozzles, valve bodies, pump casings, pipe fittings (such as elbows, tees, reducers, flanges, etc. conforming to ANSI B16.9), and pipe branch connections and fittings.

(g) examination requirements are under development.

(2) The welds initially selected for examination shall be  ;

re-examined over the service lifetime of the piping component. l (3) For welds in carbon or low alloy steels, only those welds showing reportable preservice transverse indications need to l

All terminal ends and dissimilar metal welds have been included in the Category C-F Program as required by Notes (1)(b) and (1)(c) above.

These welds are identified in the notes column of the Category C-F Program a List.

Note (1)(d) was adhered to in all cases except that Note (1)(d)(2) was not used. A total of 25% of the welds in RHR and the ECCS systems were selected for examination and included in the Category C-F Program.

A summary listing of the welds in the 10-year Category C-F Program l are included with this letter. This list should be incorporated into the i Inservice Inspection Program, Rev. 1, under Tab #31. More information on these welds (such as pipe size, calibration std., etc.) can also be found i under Tab #31.

As was done for the Examination Category B-J Program, LaSalle reserves the right to revise the specific weld sample to meet ASME Section XI requirements. Most of the welds in this category are optional as long as the minimum percentage requirements are maintained. Again, any weld substitutions resulting from inspection difficulties will be reported in the subsequent ISI Summary Report. Substitutions will only be permitted during the first inspection interval.

Augmented Inspection Type 1A requires the testing of all circumferential butt welds (and the associated seam welds) on those lines considered to be in high energy line break exclusion regions. The list of welds presented in the ISI Program, Rev. 1, contained portions of systems which are not considered to be in these regions. This addenda revises the Augmented Inspection Type 1A Program to delete those welds not in high l energy line break exclusion regions. The revised program is included and should be incorporated into the ISI Program, Rev. 1, Tab #36.

The ISI Program changes made by this addenda are in full accordance with ASME Section XI and in no way violate the LaSalle Station Technical Specifications or the LaSalle Updated Final Safety Analysis Report. These changes have been approved by a formal on site review in accordance with station procedures.

LaSalle plans to implement this addenda prior to the Unit 1 2nd Refueling Outage which is currently scheduled to start in March, 1988.
