NRC RADIATION PROTECTION INSPECTION REPORTS 50-373/98018(DRS);

50-374/98018(DRS) AND NOTICE OF VIOLATION

Dear Mr. Kingsley:

On July 16,1998, the NRC completed an inspection of your radiation protection program at the

LaSalle Nuclear Generating Station, Units 1 and 2. This inspection included observations of

activities in progress, interviews with personnel, and a review of applicable records and

procedures. The details of the inspection are described in the attached report.

' This inspection included a review of the extemal exposure monitoring and air sampling

programs, high radiation area control, and testing of the standby gas treatment filtration system.

The circumstances surrounding an April 1998 shipment of irradiated metal components to a

burial site and two high radiation area control events, occurring in March and May of 1998, were

also reviewed.

Your staff effectively implemented the extemal exposure monitoring and high radiation control

programs. In particular, we observed that administrative controls were implemented to maintain

. worker exposure as-low-as-reasonably-achievable (ALARA) and that doses to declared

pregnant workers were maintained at a minimum. Air sampling activities were also well

implemented, but some examples were identified where precedural improvement was needed,

notably with the tracking of lapel air samplers and associated sample results.

..We also concluded that your staff promptly investigated and took appropriate actions

conceming irradiated metal found inside a Type B cask, which was identified by another NRC

~ licensee. Although neither the source of the metal nor the duration that it was inside the cask

could be determined, we concluded that its presence would not have resulted in a significant

health hazard during transport.

However, based on the results of this inspection, the NRC has determined that a violation of

NRC requirements occurred. This violation concemed an entry into areas of the Unit 2 A -

reactor water cleanup heat exchanger room (a locked high radiation area), which had not been

surveyed to evaluate the extent of radiation levels in the area. Although your radiation

[

protection staff had not performed a complete survey of the area, the staff did not implement .

controls (such as postings and/or barricades) to prevent the workers from entering the

unsurveyed areas and did not adequately communicate the access limitations to the workers.

Consequently, the workers inadvertently entered the unsurveyed area. The failure to

implement these controls indicates a lack-of-attention to a fundamental aspect of a radiation

protection program (i.e., implementing proper RP barricades and postings) that could have

resulted in a significant, unplanned personal exposure.

The NRC has concluded that information regarding the reason for the violation and the

corrective actions taken and planned to correct the violation and prevent recurrence are

adequately addressed in the subject inspection report. Therefore, you are not required to

resperd to this letter unless the description therein does not accurately reflect your corrective

acdonu or your position. In that case, or if you choose to provide additional information, you .

I houlo follow the instructions specified in the enclosed Notice of Violation.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosures will be placed in the NRC Public Document Room (PDR).
