DAVIS-BESSE INSPECTION REPORT 50-346/98009(DRP) AND

NOTICE OF VIOLATION

' Dear Mr. Wood:

On June 23,1998, the NRC completed an inspection at your Davis-Besse site. The enclosed

report presents the results of that inspection.

During the 6-week period covered by this inspection report, activ.ities at the Davis-Besse facility

were generally characterized by safe and conscientious operations. The restart activities

I

following the refowling outage and the subsequent power reduction to repair a main feedwater

pump were carefully controlled and well coordinated. However, on two occasions, operators

performed actions without management approval when faced with unanticipated circumstances

after operator errors occurred._ Additionally, we are concemed about one violation of NRC

requirements that was identified.- Specifically, NRC inspectors identified that the design basis

requirement of preventing debris from plugging containment spray nozzles was not translated

into the design of the emergency sump in that a design drawing permitted several unscreened

openings around the base of the sump screen enclosure. These openings, which existed since

plant construction, were larger than the screen size openings and could have allowed particles of

sufficient slee to bypass the emergency sump screen and plug containment spray nozzles.

This violation is cited in the enclosed Notice of Violation, and the circumstances surrounding the

violation are described in detailin the enclosed report. Please note that you are required to

respond to this letter and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to determine whether further

enforcement action is necessary to ensure compliance with regulatory requirements.
