(NRC INSPECTION REPORT NOS. 50-327/87-60 AND 50-328/87-60)
This refers to the Nuclear Regulatory Commission (NRC) routine inspection
conducted by K. M. Jenison on September 6 - October 5, 1987.
The inspection
included a review of activities authorized for .your Sequoyah facility. At the
conclusion of the inspection, the findings were discussed with those members of
your staff identified in the enclosed inspection report.
Areas examined during the inspection are identified in the report. These areas
included the Restart Test Program and the Startup Activities List (SAL) Closure
Process.
Within these areas,
the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel, and observation of activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
Three of the identified violations involved:
(1) an
inoperable Unit 2 hydrogen monitor since plant licensing;. (2) operation of
essential raw cooling water (ERCW) without procedures; and (3) modifications to
the ERCW system without a safety evaluation.
Accordingly, the violations,
references to pertinent requirements, and elements to be included in your
response are described in the enclosed Notice of Violation.
One additional violation (50-327,328/87-60-02) was identified involving a
failure to meet 10 CFR 50.59 such that a required license amendment was not
sought.
This additional violation, which is described in paragraph 3 of the
enclosed inspection report, is under consideration for escalated enforcement
action.
Accordingly, a Notice of Violation addressing this particular
violation is not being issued at this time, and therefore no response to this
violation is requi red.
However, please be advised that the number and
characterization of violations described in paragraph 3 of the enclosed
inspection report may change as a result of further NRC review. An enforcement
conference to discuss this violation was held on October 22, 1987. You will be
advised by separate correspondence of the results of our deliberations on this
matter.

Three unresolved items were identified concerning:
(1) hydrogen monitor
operability for Unit 1; (2) operability of mechanical sleeves in polar crane
wall penetrations; and (3) resolution of recently identified security issues.
These matters will be pursued during future inspections.
Your attention is called to the commitments made by TVA during the exit
interview.
The first set of commitments (five in total) involve containment
spray pump 2-A.
The second set of commitments (two in total) involve TVA's
SAL closure process.
Both sets of commitments are addressed in detail in
paragraphs 3 and 18 of the enclosed inspection report, respectively.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter, its enclosures,
and any reply will be placed in the NRC's Public Document Room.
The responses directed by this letter and its enclosure are not subject to the
clearance procedures of the Office of Management and Budget as required by the
'
Paperwork Reduction Act of 1980, PL 96-511.
